You can mention at last in bullet points. I used to mention my gpa, scores, and published paper in bullet points at last. Will be easier to get noticed.

Email lots of professors that’s the only thing you can do. It ll increase chances
If you change your surname based on marriage can you write ur current surname on the letter since it is different from the one on your certificate
I think the name should be same as in the passport because that’s the one main document here. But you can do more search to make sure.
