I'm trying to figure this out on my own, but I was wondering if you had any suggestions. Your solution is very good for filling out documents that are to be printed and filled out using pen/pencil. The problem I have is when someone tries to type in their name for example, it doesn't keep the line underneath the inserted text. I was thinking of using developer tools for fields, but I'm not making much progress with that.
jairtzinio You need Adobe Acrobat Creator to specify fill-in-the-blank fields. For Word, you use Content Controls to create fill-in-the-blank items. I don't know whether they translate to the PDF document.
i just tried it by converting the file to pdf but it was disastrous, so it looks like im just better off filling it out and scanning it. im not sure if its because it didnt convert well but im not saying its not possible but for some reason it doesnt seem to like it as its horribly unresponsive when i try to fill out the form. mind you i have a good computer

very helpful, however, Cereal should be Serial if you are wanting a number and not breakfast food.
