  • Here at CataractCoach we have long extolled the virtues of MSICS: manual, shelved incision cataract surgery, where we make a larger scleral incision and remove the nucleus whole in an extra-capsular manner. This is the modern (and far better) version of the old ECCE surgery: extra-capsular cataract extraction that was taught decades ago. The main advantage here is the incision which is so nicely shelved that the long tunnel length allows for an incision that seals on its own. Though I like to place at least a suture or two of 10-0 nylon to secure the incision, some high volume MSICS surgeons do the procedure sutureless. This video demonstrates that technique and no sutures are used to close the incision or the conjunctiva. If you have never learned MSICS, I encourage you to do so -- it is easier than you think and I even have a four step video that teaches the method. If you have not learned MSICS, why not?

    Thank you Dr Devgan for featuring my video. It means a lot. I am taking all your inputs to make the surgery better. The IOL was dialled in multiple times as the trailing haptic was refusing to go in.

    Great technique
    Though I prefer visco expression of the nucleus to the vectis....much safer in my hands

    I think your hydrodissection is a bit risky (herniation of the iris) when you do it through the main incision
