    just wondering would u normally biopsy an isolated hematuria with Urine PCR only 300mg/g and normal creat ??

      I think most nephrologists typically would not. You should rule out urologic causes first. If the kidney is implicated, the differential would be very mild IgA vs TBM vs normal kidney with that scenario - all 3 would probably just be monitored conservatively. So I would tell the patient that a biopsy is unlikely to change the management, and typically would just recommend close monitoring of the urine PCR. If proteinuria goes up with time then a biopsy may be indicated.
