Excellent case!!
A true head scratcher as you said. I thought of post infectious GN (because of pacemaker) but no humps, also of AIN but Immune complex deposits ruled that out.

Hi just went through the discussion - I was having all along diagnosis of IRGN/PIGN.
Elderly presenting with pulmonary edema and renal failure , most imp diagnosis to rule out is PIGN
All the symptoms and lab criteria fit to this
It’s more of RPGN - class II
