I would like to point out that the Shane/Jericho spot that you said "went wrong" wasn't an accident. Shane was hitting Jericho for real throughout the match and Jericho had enough. It's not the first time a wrestler got revenge in the middle of a match. Brock also did it to Braun. After Braun was giving him stiff shots Brock decided his next few punches and knees would connect for real.
