    Hi Patty, I am so grateful to you for posting these videos. It has meant the world to me these past 6 weeks as I have studied for my CNA test that I took today. (I passed-Yay!!!) I appreciate your generosity in posting these videos and sharing your knowledge with us; that is the most selfless thing a person can do. I hope your kindness comes back to you tenfold. Your videos helped me pass my first time taking the test and to those reading, if you are testing in FL, watch these videos!

    Yes, you may roll the patient over to place the bedpan under them, but for this skill the patient is able to lift their hips and many students find that process easier. Either way is acceptable for the exam (it is based on the patients level of ability and since your "patient" for the exam is another testing student, they will have that ability). Thanks and good luck!

    The state requires that cleaning the patient's hands is done AFTER removing, emptying and cleaning the bedpan but BEFORE washing your hands. Since you really don't want to contaminate the hand wipe container with the patient's urine on your gloves, you should wait until you have removed your gloves before giving them the hand wipe. The faucet is the dirtiest thing in the bathroom, so you can touch it with your dirty hands or dirty gloves. Use a paper towel if your hands are clean. Good luck!

    @85Dignity Thank you for your comment! When performing skills, we need to be mindful of proper infection control techniques - part of which is keeping our gloves clean for the patient. Therefore, it is a good practice to apply your gloves immediately before touching the patient to keep them clean. As long as your skin is intact and there are no body fluids on the sheets, this is the appropriate time to apply your gloves! These principles are reviewed in our online program!

    Very good question! If the testing center doesn't have disinfectant spray, you will perform all other actions as described in the video, omitting that step. Because most of these supplies will be used on multiple testing candidates, there must be a way to disinfect those items for future use, so there should be disinfectant spray available for you to use. Thank you for your question!

    Yes. The skill is presented in a specific order because of infection control principles. You must close the curtain for privacy BEFORE washing your hands because the curtain is not clean and will contaminate your clean hands. You must put the barrier down under the patient BEFORE placing the bedpan. You must put your gloves on right BEFORE touching your patient to keep your gloves clean for the patient. The order of each step matters. Good luck!

    You will definitely need to put a chucks under the bedpan because with the movement of the head of the bed, the contents can spill and soil the linens. Taking the chucks out with the bedpan gives you a way to cover the bedpan for transport. I like your rationale about leaving the chucks in place in case the patient couldn't wipe well but for the test, the care plan will state the patient can wipe themselves, so that is not necessary. Good luck!

    Regarding the infecction control principles, you should NOT touch the call light or bed controls with soiled gloves, because you will further soil those surfaces and the patient touches those surfaces, which can cause infections and illnesses. Remove your gloves (or use a barrier between soiled gloves and surfaces) before touching those items. Put a second set of gloves on to remove, empty and clean the bedpan then remove those gloves before touching any surfaces. Thanks!

  • @4yourcna

    @85Dignity Thank you for your comment! You are very right, every state is a little different and every teacher will teach a little differently, as well. Good infection control comprehension is important to keep your gloves clean - so everything you touch with gloves on will contaminate the gloves before you even touch the patient. It becomes important to understand which items in the patient environment are clean and dirty...and only applying gloves before touching patient! Thanks

    Yes, this procedure takes several minutes, but every step is necessary. Failure to perform all steps as shown can result in a failing grade for the exam and complications for the patient. This video demonstrates one method (the preferred method for the exam), but other methods can be used. If the patient cannot lift their hips, they can be rolled to the side and the bedpan placed against their buttocks and held there as the patient rolls onto their back. Good luck on the exam!

    Disinfect per your facility policy. If your training center told you to rinse three times, then perform the skill that way. However, testing centers should have disinfectant spray as the bathing items are used by multiple people during testing and should be disinfected properly to prevent cross-contamination. I am concerned about the lack of infection control principles here and would ask the evaluator if disinfectant spray is available. Good luck and thank you for your kind comment!

    Thank you for your reply! Your response was absolutely correct! However, we removed the comment you were responding to due to the profanity and unprofessional nature of the post made by the other individual. So, for anyone else reading this post, the question was regarding why tissue was given when most patients cannot wipe themselves. Promoting independence is the answer. If the patient is unable to perform any task, we will assist them as necessary. Thank you for your comment and good luck!

    For the exam, you are not required to raise the bed to a comfortable height because not all beds in all settings have this ability (think home care and assisted living). You may adjust the bed to your comfort if you wish but MUST lower it at the end of the skill (safety). These videos demonstrate current testing principles only and that step is not required. I explain this thoroughly in my classroom training - these videos are intended as an adjunct to classroom training programs.

    All nursing healthcare professionals (CNA, PCT, LPN, RN) must be familiar with how to perform personal care skills (including bathing, toileting, feeding, etc) and be able to perform these skills for patients that cannot do so for themselves. So, yes, all of them will, at some point, be dealing with body fluids. Having the ability to remain professional while performing these skills is important. CNAs will, however, spend more time with the patients and will usually do more personal care.

    The bedpan was cleaned and disinfected prior to storage in the bedside cabinet, therefore it does not need to be handled with gloves until it has been placed under the patient. However, you may apply your gloves at this point if you wish. Please be careful not to contaminate your gloves by touching anything "dirty' in the environment once you have applied your gloves but before touching the patient to prevent contaminating the patient. Thanks and good luck!

  • @4yourcna
    @4yourcna  11 років тому

    Yes, the patient should be given something to wash up with after using the bedpan (either a soapy washcloth or a hand wipe is sufficient). Certainly, having the patient wash their hands at the sink with soap and water is the best option, but the patient is unable to leave the bed (or they wouldn't have needed a bedpan in the first place), so we have to make do and help them clean their hands in the best possible manner. Good luck!

    Any skill where the patient may be exposed in any way requires a privacy blanket. Think of how you would feel lying in bed in a hospital gown with no sheet (cold and exposed) so anytime we remove the sheet, we must place something over the patient.

  • @4yourcna
    @4yourcna  11 років тому

    Thank you for your comment. This video reflects current Florida state standards. Cleaning is only required if the patient cannot do so themselves. The care plan for this skill states the patient is able to complete that task on their own, so cleaning is not necessary. Thanks for the comment and good luck!

  • @4yourcna

    Breathe. Take your time. Remember that they are not looking for perfection, but the test is primarily there to determine if you are able to perform skills safety and have a good understanding of patient rights and infection control. You may correct any mistakes you make verbally ("Correction, I would have...) unless the evaluator stops you to point out a safety problem. Talk to the patient, explain everything. Wash hands before and after each skill. Follow the videos. Good luck!

  • @4yourcna
    @4yourcna  11 років тому

    For the exam, there won't be any stool to examine. This is a simulation skill. While in a clinical setting it is important to evaluate the contents of the bedpan, this is not a testable point on the CNA skills exam. It is covered in my classroom-based course. These videos are intended for use as an adjunct to a classroom based program and will not contain all theory or clinical principles covered in training. It is a demonstration of testable standards only. Good luck.

  • @4yourcna

    Regarding side rails in general: Side rails are not graded for the exam. Siderails are not used in many settings (such as home care, assisted living and nursing homes) and the CNA must know how to do the skills without them safely. Regarding this skill: the patient remains in the center of the bed so side rails aren't even necessary at all. You need two sets of gloves: one set to place the bedpan and one set to remove the bedpan and empty. 6 sets of gloves are not necessary. Good luck!

    You do not need a paper towel to turn the faucet on. The faucet is considered dirty since it is touched with dirty hands. Since your gloves are dirty after handling the bedpan, it is acceptable to turn the faucet on without a paper towel. You only need a paper towel to turn the faucet off AFTER washing your hands to prevent re-contamination. And yes, males use the bedpan for bowel movements but the test scenario is a female using bedpan for urination (she can undress and wipe herself). Thanks!

      +4yourCNA Hey patty... its me again :) im so sorry im going to be asking you questions a lot today on different skills since im testing tomorrow.... my question here is, I was taught in the refresher course to replace the dirty bed pad with a clean one when removing bedpan and pad. I dont see that as a checkpoint on the checklist... but want to make sure, since the instructor said that is required. Again thank you so much!

    • @4yourcna
      @4yourcna  8 років тому

      +Emily Williams It is not required to leave the patient on a bedpad. It would have to be reflected in the careplan if required. You should not use a bedpad routinely as it is plastic lined and will trap and hold heat, which can make the patient sweat and create a warm, dark, moist environment which can provide an ideal environment for bacterial growth. CNAs must follow the care plan, the whole care plan, and nothing but the care plan. Since the care plan does not instruct the CNA to leave the patient on a clean pad at the end of the skill, it should be avoided. Hope this helps.

    You will create hospital corners anytime you have an untucked sheet on the foot of the bed. The purpose is to keep the corners of the sheet off the floor. Since there is no reason to untuck a sheet during any other skill, you will not have corners hanging down to worry about. All other skills that require a privacy blanket will have you pull the sheet down toward the bottom of the bed, but will not require you to completely untuck the sheet. If you do, you must re-tuck at the end and do corners.

    Regarding your hair, it really depends on the setting you are working in. Some settings are more strict than others. In general, if you have hair long enough to swing forward when you are bent at the waist and is long enough to touch your patient, it should be secured in such a way as to prevent it from doing so. Ponytail, braid, bun, etc would work.Some facilities do not enforce this and allow long, loose hair. Your hair will bring germs home with you, so it shouldn't touch patients. Thanks!

    Yes, you may use soap, if you wish. All that is required is for you to rinse the bedpan (or any other basin) with water for the exam. I show using a disinfectant because this is becoming the standard in many clinical environments to ensure that supplies are clean when storing which helps with infection control. Disinfectant spray or soap is extra and not required for the exam. We wish you the best of luck in your test tomorrow! Let us know how you do!

    @L8DKDNA1 Thank you for your comment. For the Florida exam, it is not required to check ID bands during the test because not all patients in all settings will have ID bands (ALFs, some nursing homes and home care will not use them). These videos are demonstrating current Florida requirements. Talking each step through will inform your patient of what you are doing each step of the way and help them become more comfortable. This is reflected in the category "Indirect care" during the test.

    @dreamchasers19 It is not an infection control violation to touch a clean bedpan with clean hands. You should certainly have gloves on before touching a bedpan that has waste products in it. But the bedpan is cleaned and disinfected before being stored in the drawer (never put a dirty bedpan away), so the bedpan is clean when it is removed from the drawer and can be picked up without gloves. Our online program explains what is clean and dirty in an environment for better understanding.

    All procedures must be performed as demonstrated in the video. Failure to provide privacy, wash your hands or place a barrier under the patient can cause infection control issues or a serious violation of patient rights. Healthcare settings can use electric beds (press a button) or manual beds. Most people know how to use the electric beds, so we demonstrated this skill on a manual bed so that students would have an opportunity to see how they work using proper body mechanics. Thanks!

  • @4yourcna
    @4yourcna  11 років тому

    For the exam, it is not necessary to contain the bedpan in a plastic bag. The bedpan has been cleaned before storage, so there is no need to wrap it in plastic. It must be wrapped in an absorbent material (the chux) when transporting to the toilet for disposal. You may certainly clean it with soap and water if you wish, but it is not required. Simply rinsing and disinfecting is sufficient. Thank you and good luck.

  • @4yourcna
    @4yourcna  12 років тому

    You have to provide privacy for this skill. Using the sheet for privacy may get the sheet wet or soiled, requiring a linen change. It is better to use a privacy blanket and pull the sheet down to the bottom of the bed. You can roll the patient onto their side or have them lift their hips to place the bedpan under the patient (method does not matter), but if rolling the patient, make sure the patient scoots toward you before rolling so they stay in the center of the bed for safety. Thanks!

    Thank you for your response. My teacher, teach us that before roll the parient to other side of the bed we should roll them first towards us in 3 section ( shoulder, hips and legs) to place the bed mat. And do the same to other side. I am waiting for my florida exam schedule.

    • @4yourcna
      @4yourcna  8 років тому +1

      There are many acceptable ways of performing the skills. I cannot remark on whether your instructor's technique meets testing standards but I can tell you that generally it is not acceptable to roll a patient toward you as it is poor body mechanics (the bulk of their weight along their shoulders, back, hips and buttocks is far away from you) and your patient should never be positioned eye level with their crotch. This video reflects one acceptable method of performing this skill which meets testing standards. Best of luck!

  • @4yourcna
    @4yourcna  11 років тому

    For the exam, you are not required to clean the peri area as this skill will be demonstrated on a fully clothed student and the care plan states they can clean themselves. So you will not be cleaning the patient, therefore there is no reason to set the bedpan down. In a clinical setting, you may set it on the floor as long as it is on a barrier (not directly on the floor) and remains covered for infection control reasons.

    The gloves are removed before elevating or lowering the head of the bed so that you do not contaminate the bed controls with your dirty gloves (the patient touches that bed control with their bare hands and it should not be contaminated by your gloves). You do not need to wash your hands in between because you do not leave the room or the patient. Hands are washed at the beginning and end of the skill only. Please view my instructional video on this skill for more thorough instruction.

    You may do the skills how you wish, both are appropriate and effective. Our videos are researched, prepared and presented by an RN (me) with over 20 years experience. We were also a testing center and active in nursing education. This technique is appropriate and more in line with clinical practice. You will not be an effective employee in a clinical setting by washing 3 times for each skill - this is unrealistic and takes way too much time. The test doesn't even require disinfecting!

    For the exam, you can ask them to raise their hips (they will be able to do so for the exam). If, in real life, the patient is not able to raise their hips, put the siderail up on the opposite side for safety and roll the patient onto that side to place the bedpan under the patient, then roll them back on top of the bedpan, so they remain in the center of the bed on the bedpan. If no siderail is available, the nurse will be able to determine a safe plan based on the patient's abilities. Thanks!

    You may if you wish, but if you are not leaving the room or touching anything other than that patient's equipment, it is not necessary. Since you are only adjusting the bed and putting on another set of gloves, you don't have to. But you can, if you wish. It will not count either way. Good luck!

    I am not specifically familiar with California testing standards, but these skills demonstrate basic nursing principles which are universal. The actual skills required to be demonstrated may vary slightly (the patient may be in a bed for feeding, etc), but the performance of the principles contained herein should be similar. You should review the specific standards for your state (google ca cna exam clinical checklist and look for website that starts with asisvcs). Good luck!

    The bedpan (for the exam) is unused and will be cleaned and disinfected before being stored, as such it is not a fomite. You may certainly place it on a protective barrier, but this is not a testing standard as the evaluator and the test administrators understand infection control principles and do not consider the bedpan a fomite. You must wear gloves before touching a USED bedpan, certainly. But a clean bedpan is clean and requires no gloves- You may wear gloves if you wish.

    Putting baby powder on the bedpan can be useful to keep the pan from sticking to the patient's skin, but this cannot be used in patients with a respiratory condition, as the baby powder is very fine and easily inhaled, which can cause a respiratory crisis. You must make sure that this step is allowed by consulting the care plan or the nurse. It is not required for the state exam and does not have to be mentioned during testing. Thanks and good luck!

    For the Florida state exam, the "patient" is able to remove their own clothing and wipe themselves. You are only responsible for placing and removing the bedpan (and protecting the bed with a chucks) and emptying and cleaning the bedpan. All other actions are performed by the patient. In a real-life clinical setting, you will perform any actions the patient cannot perform on their own (undressing, cleaning, dressing, etc). Hope this helps!

    I am not familiar with Texas state standards, so I do not know what points are different. These videos represent current nursing best practices (which are universal) and should only vary in specific scenarios presented for the exam. I wish we could be of more help but wish you luck!

    @4yourCNA Im in california and here we dont have to say every single step we are going to do but we do have to explain the procedure to the patient and guide them through it. We dont have to say "Im going to close the door"..." Im going to throw this away" ect... and here for the test we do have to check the arm band to make sure its the correct patient and of course there are different places were the patient wont have a arm band but we test here as if we were in a facility.

    I am sorry, but I don't know if CT is still testing on blood pressure. Best advice I can give you is to learn it and practice it, just in case, because you need to know it for your career anyway! :)
