Once you mentioned Rosemary's Baby, then every person in SON was suspect as being part of a cult. Hence, no real surprise at all with Paul.

The scariest part of this movie is the medical bills she would have.
Tbh would have been waived since they were apart of the cult. Worse case if she lived, she can sue for malpractice cause she has proof she's been trying to get in contact and that the doctor didn't believe her

Then it is never mentioned what she does for a living or if she even works anything at all, but they live in a nice house? Which obviously is in her name since she plans to sell it. They could have at least make her character a writer or something.
Why did the detective even go to talk to the doctor if he is a part of the cult and knows the truth? Why stop her when she summoned the demon?
The twist in the end was actually pretty unexpected

I knew as soon as the bandaged junkie cut but I suspected him the first scene saying he believes her. But yeah the movie plays it pretty close to the chest. I was pretty sure she wasn't insane and that it was real but it still made for some intense doubts

I knew Paul would be a part of the cult. Nobody goes along with that much for that long. It's also just obvious.

She wasn't giving him to the demon. She was gonna say make him a normal boy or I'll kill him. The carnival was like one last thing she could do with David before she went to do this.

The "twist" was actually an easy guess early on. Not bad. It was well written, acted, directed, and all around crafted. Very enjoyable. It's just a predictable trope by now. The moment he said "I believe you", I said "he's in on it." It's honestly more realistic to start off with doubts.

I dunno if I'd call Paul's being a cultist a twist. I had him pegged the minute he showed up on screen.

In the beginning when Paul sent his partner away then told her he believed her, I knew he was part of the cult and didn’t trust him from then. It was so apparent to me, it was hardly a twist.

Might be pointing out the obvious here, but i feels like the part where the kid eats the neighbour is a reference to the iconic scene in resident evil 1 where the first zombie you encounter is eating a corpse.
Yeh Paul fed him when he mysteriously got better the first hospital visit, good twist at the end. Enjoyed it.

I disagree with you on the idea of it being her mind that causes the various “episodes” we see, but that of the demon, the entity clearly is possessive of his son, but seemingly can’t interact much physically with this world. It would make sense for both the cult and the demon to be working in tandem to hasten the end times.
Not to sound rude, but at this point, the actual plot twist would be if the "compassionate loving" character would turn out to be genuinely good. It's become a cliche, imo.
