Thank you for the good instruction and for explaining the differences between plantar fasciitis and damages to the fat pad. Very educational as I've recently got diagnosed with both conditions in the same heel. Just started taping and hope to get rid of both damages!

would you recommend staying off of it to let it heal, like with hands free crutch, or staying on it, working it out?

A combination of those. Rest and offload taping!

I have had this now for 2 months. Was diagnosed by ultra sound indicating plantar fasciitis and heel spur. Later a chiro advised me correctly that it was fat pad syndrome. This is exactly the pain I was feeling...extremely tender and painful...after racing in spikes in track & field. Do you have exercises to recommend?

Unloading with a taping technique to offload and gather tissue would be recommended. This could be done with Leukotape or Dynamic Tape!

I also have a burning pain along the side of the fat pad when my feet are in the ground. If I tape it, the tender pain in the middle of the heel gets better but the burning pain along the sides get worse.

You may need a different type of treatment or taping.

I was taping for several weeks at the advice of my PT, but then I developed really bad eczema and 3 weeks later and a couple of tubes of Hydrocortisone cream later and it's still not gone (it's slowly getting better.) I NEED the tape. It is the only thing that significantly helps. The gel cup that the PT also told me to use doesn't do much. I'm about to start a new job that will require a lot of walking. Is there anything I can do to prevent the eczema from coming back once it's gone? It wakes me up at night.

The skin needs to breathe, so remove tape every 4-5 days for at least 24 hours

I’ve had chronic foot pain in the ball of my R foot and general soreness in both feet when standing for basically only a few minutes at a time. Multiple doctors , bloodwork and scans done. No diagnosis yet. I’ve noticed that on my R ball of foot there seems to be significantly less padding than on the L foot. I can feel every bone very easily. It gets very irritated throughout the day as if I’m walking on a giant lump. So far all the docs I have seen say that “it looks fine and shouldn’t be causing issues”. Is there any fix for loss of padding on the ball of your foot? Im only 33 by the way and not a runner of anything like that. So it either vanished one day or it’s just a coincidence that I have less padding on one foot. Appreciate any input!

When pietzogenic papules occur just before heel pad pain occurs, does that mean the fat pad has escaped out through hernias into the papules and cannot be taped back in?

The taping is designed to manage load. The healing is dependent on multiple factors.

    Hi Dr Vick,
Can you kindly help me? I've this painful heel for nearly 2 years after striking my heel on the ground while riding the kick scooter. I always thought that I have plantar fasciitis and continue the massage and rolling roll treatment but it does not seem to help. I am now beginning to suspect that I have a fat pad injury instead. Will massaging and rolling on wooden pin actually make the heel pad injury becoming worst? Thanks.

Hi Leslie, this type of self treatment could make your condition worse. Please consult with a certified sports chiropractor in your area! (CCSP) They will be able to assess and address your pain. Dr. Wilhelm - Bozeman MT Certified Sports Chiropractor

This describes exactly what I am experiencing. My Orthopedic gave me a shot of steroid in my heal but didn't even mention Fat Pad syndrome or taping. He said if the steroid shots don't work after about 6 weeks, he will cast it so I can get the weight off my heal. To me that (casting) sounds like too much too soon. I will tape it and try the heal cup and see how that works. Thanks for the advise!!!

+Rex H shooting somebody with cortisone without a MRI or x-ray is not a good idea. Steroid shots can dissolve your fat pad and if You already have fat pad atrophy steroids will do absolutely nothing for that.I'm speaking from 25 years of experience with foot pain and the best thing you can do when you first have it to stop all athletic activities for a minimum of two weeks and get a proper diagnosis backed up by MRIs or x-rays preferably if you have access to infrared medicine they can diagnose soft tissue maladies within minutes using an infrared camera.
