    I was using the Libre 3 with multiple injections and just got the Medtronic pump and thus switched to the Guardian 4 sensor. When I first switched, I still had life in my Libre and so kept it on to compare. When I did finger sticks, the Libre 3 was about right on with my finger sticks, while Guardian was at times 15-35 off. I will say that the Guardian got better with time, but they only last 7 days vs. Libre’s 14 days. The Guardian is also so complicated to put on. I need a cheat sheet to make sure I don’t miss a step. Being new to pumps in general, I can’t compare the size and tubing for the Medtronic pump, but I will say that I am amazed at how it is learning me and my overnight numbers are stable and helping me sleep so much better. But more than anything, the Medtronic nurses and reps are AWESOME! They are so kind and patient and there for me even if I am stressing out. Knowing I have someone I can text anytime I have a question is so reassuring and has made all of this switch worth it. Even if the Guardian sensor requires me to have someone help me get it on since I can’t seem to get it on my arm one handed. If I could have a Libre sensor with the Medtronic pump and staff, it would be perfect! (I had a great experience with the Abbott customer service too, so win, win!)

      I want to update my comment since it has been a couple months. I am now able to apply my Guardian sensor to my arm, pull out the needle, insert the transmitter, and apply the overlay tape all by myself with one hand. My time in range is pretty good, typically between 86-97%. And I just feel so much better not swinging from 300s to 50s. The Medtronic reps have continued to be great and have been able to advocate for me with my endocrinologist when I needed my carb ratio adjusted. I sometimes wonder how accurate my readings and thus my time in range is, but it is never off enough that it would affect how I treat. Just that mid/high range - example, I may be actually 187 instead of 160. Overall, the autocorrections of the smartguard have been a game changer for me, as well as getting my basal doses all throughout the day instead of one nightly injection. So thankful for technology!

    Hi Justin thank you so much for all your hard work in keeping us T1 diabetics informed. I am going to order my 5th pump and going with Metro nic MiniMed 780G pump. I will never give up my Libre 2 cgm. I'm so happy that the 780G pump will be able to recognize Libre2 and Libre 3.
    T1 diabetic for 66 years now and still healthy!!! 😀

      I can't even imagine what would be possible with the G780 + Libre 3.
      I use the Gurardian 4 and that's just bad compared to the Libre sensors.

      You don't have an old pump lying around that you can sell me? I am just asking for one that is compatible with AAPS, OpenAPS, Trio or something that is compatible with a DIY Artificial Pancreas Closed Loop system - 40 years into diabetes, looking to get where you are while healthy still :)

    I am a current user of the Guardian 4 / 780G system. I think the pump is great, I think the algorithm they created is second to none, but the guardian 4 cgm is total crap. Not good at all.
    I was also a part of the Dexcom G7 / Omnipod 5 integration trial and that was fun but the timerframes to change everything over didnt match up since you only get 3 days out of a pod but the G7 lasts 10 so if you want to change everything at once, you either have to lose a day on a pod or a cgm and insurance isnt going to send you the.extras to cover that loss every month so i went back to the 780g. Once i can get my hands on the simplera, i think everything will be fine.

    I am a user of a 780G / Guardian 4 close loop system, it works quite well. But there is a BUT... The installation of the CGM is quite tricky... I wait for a more easy to use CGM, Simplera or Libre 3/4.

      The guardian 4 is a pain to use with all the tapes etc and falls over a lot

      Have no idea why it needs to be so complicated. I've just started using it and it's such a pain in the ass to insert. Shame, because the actual CGM interaction with the 780 is working quite well so far. My country hasn't approved the Simplera yet

      @@superwhizz114 I have been said a few weeks ago by the nurse in charge of helping me with the pump and the CGM (it's compulsory here in France to be followed by a professional ) that the sSmplera is to be available in 2025... Let's hope !

      As I wrote on Medtronic's LinkedIn post about this cooperation, it has been a huge waste of time, money and human resources developing Simplera, when they could have opted for Dex or Libre 3 directly. I guess it's the big company ego that pulled the strings.

    I currently use Guardian 4 and it the worst CGM I have ever used. I am ready to stop using the 780g because of it.

    Hi Justin thanks for all the hard work in keeping us informed!!! I am on the 780G pump with the guardian 4 sensor! hate it 7 day wear and YOU STILL HAVE TO DO ONE BLOOD SUGAR FINGER POKE!!!!! When the sensor finishes its 2 hour warm up. go with all that I still do love the 780 g pump and algorithm, it keeps me with in range but starting to hate the tube and the charging of the transmitter and the 2 hour warm up and all the over lay tape on the sensor...the libre system and 780g pump sounds like a wonderful partnership. I hope it works, but in the mean time I am on the works to get on the LOOP system with omnipod dash and dexcom start up next month!!!! and then when omnipod 5 comes out (Canada) I want to switch over to that. Now if Medtronic came out with patch pump with 780G algorithm and libre system in the future and depending on how omnipod 5 works with me I would have a hard time choosing, BUT not right now. been with medtronic for many years and scared of change my A1c from 12 to now 7 had 6 once or twice. So there is history there but i need a more comfortable, affordable, stress free future.

    I use Medtronic 780 with the Guardian 4 and as all have said the issues with the CGM are the size, all the tape needed to hold it and the 2 hours to start up after changing. The algorithm is great and gets better.
    You asked about sharing and Medtronic allows you to share the pump info from their website. I use this to share with my Dr. so that they have my data as soon as I am there.
    You either have to upload it from a computer with a dongle that they give to connect it or what I use is the android app which uploads my data every night.
    From the site you can have others be notified if lows or highs happen but I never used that function so not sure how well that works.

    Good move. I've been using Libre 3 for over a year, pretty happy with the sensor but Android app kinda sucks.
    Medtronic needs a smaller discreet pump.

    I have only ever used Medtronic pumps. I have used both G3 and G4. G4 is much more accurate and does not require calibration which is a huge plus. My only dislike is it is big and clunky.

    I hate the Guardian 4 sensor with a passion!!! If I could use a Libre sensor with my Medtronic pump, it would be fabulous!!!!

    Medtronic Guardian has sucked forever and wasn't made to be for everyone. I worked at Medtronic Minimed and it was interesting to be on the inside and understanding why things worked the way they did. I couldn't stand the guardian system, because if you missed a calibration or you fell out of range it would fail the sensor and you'd skyrocket. Knew a few people who had the same issues. I've had great luck with Libre 2 & 3. My son has used the Dexcom G6 & G7. Dexcom is the absolute worse for him (constant failures). Freestyle has worked for him as he is on the tandem pump.
    I love Medtronic pumps and probably will never switch. This news was on my radar for a while and i'm super excited to see how it'll help me.

    I am a very happy user of the 780g with Guardian 4. Mine TIR over the last 30 days is 93%. For all who have problems with inaccuracy the first day do the following. Insert the sensor about 12 hours before you connect the transmitter to the new sensor. Since I do this I have almost zero accuracy/update problems.

    I'm using the 780g/Guardian 4 closed loop system. It's fantastic. Unlike many other systems, you can actually set the target to 100mg/dL which I believe is the lowest in the industry. Other systems like Tandem's only down to 110mg/dL. In my case, the system has proven to be extremely accurate but this is very subjective. One thing that certainly needs improvement is the installation system since always putting tapes to keep it in place leaves marks on the skin and seems antiquated compared to Libre's CGM. Another thing that needs improvement is the 2hr warm up period which is way too long. But probably the best reason to stick with Medtronic is their customer service. I tried calling Tandem a few times and just ended up frustrated with their unresponsiveness and big line Qs. They come nowhere close to Medtronic's team.

      I agree with you, I know two persons they using 780 with but they had a problem with Guardian 4 ! It’s alway disconnected alway updating then it’s give them a signe to change sensor they ended with stopping sing the 780 Medtronic.. in my country we pay just around 900 US dollar to get the pump and they return the money if you stope using the pump .. I believe they have an issue with there sensors.. for me it work fine but sticker and inserted really need to change it

      @@type179 In my experience, this is mostly related to bending of sensor strip. To solve this, never sleep on the arm that has the sensor and try not to smash it against surfaces.

      My problem with the guardian 4 sensors is after the 2 hour warmup it goes to sensor updating for hours (well over 3) I have to disconnect & start again, sometimes an additional twice before the sensor will work. No one can figure out why I have this problem.

    Thank you for including the information for different countries. It can be confusing to get details about what's happening in different places, so I appreciate the effort. I've never yet used either Medtronic or Libre but I'm interested to see where this goes for them.

    I’m 780g/guardian close loop. Last Hba1c 5.7, and the close loop system certainly erases smaller mistakes and rounds some edges.
    I hate the guardian sensor though. Long charge times, bulky awakened shape. High fail rate. Etc.
    But it all works well. So… there’s that.

    Other than what people have said already about the guardian 4 sensor, the frequency is every 5 min and the new libre cgms are every minute which would be a big upgrade with going into sports/active mode.
    Another thing we get with guardian 4 that isnt available with libre sensors right now is blood prick calibrations. But this is good news.

    G4 is a real P.I.T.A.!!😤 From size, insertion, secure it to skin, pairing, charging, duration, and loss of communication.
    Had Libre, Libre 2, and Libre 3. This last one has been the Holy Grail. While the first two Libres had a few issues, Libre 3 works flawlessly. Since moving to Medtronic's 780 pump, I've been missing the Libre 3 every minute.

    Thanks for this great video. Been using Libre 2 for many years, has been very comfortable, specially for cardio exercise and being able to set it on my garmin. Now, being in 70% time on correct range, I'm about to try 780g with guardian 4 to help night cgm but... Libre 2 its been sooo good that I dont know if its worth it seeing several users comments

    I’ve used both Freestyle and the 4 Medtronic, will be transferring to Simplera Q1 of 2025.
    - Abbott works fine. Only issue is that the adhesive can come loose or you loose a patch when you hit a doorpost for example. Accuracy is fine.
    - The Medtronic Guardian sensors are just okay. Seven day wear-time is acceptable but not great. The accuracy is less than Freestyle libre and usability/ ease of use is simply bad. On the other hand it couples to the 780G system which is simply great, so for that reason I put up with it.

    I was using Dexcom G7s with my 780g last year before the firmware update actually made it a 780g (so I guess it was a 760g, is that what the model was?). Using the Guardian 4 now, and as others have said, the closed loop functionality is better than manual even with a CGM. But man, the taping and setup of the Guardian 4 is so barbaric compared to the Dexcom sensor. The dual taping and clipping in the transmitter and needing to recharge it every change, it definitely feels like they’ve lost the race right now.
    Plus, I get a surprising number of failed sensors, like I’ll have it on for 3 or 4 days, sitting at my desk at work having had really good accuracy up to that point, and the pump up and starts beeping with a, “Change sensor now,” message. At least Medtronic is good at sending replacements for free, give how much these things are, even with insurance.
    Definitely looking forward to Simplera for nothing else than easier insertion, and I wouldn’t be surprised if Medtronic just decided they can’t cost-effectively cut it as a sensor manufacturer the way Dexcom and LIbre are going.

    Who wants Simplera with a wear time of only 7 days compared to Libre sensors that wear up to 14-15 days!? Medtronic needs to stop dragging their feet and get on the ball!

    I'm a type 2 that just started insulin this year due to my autism and issues with my pancreas having issues producing enough insulin. I'm using daily injections now, but I'm also considering a pump. The Libre CGM system has been a game changer for me.
    Libre 3 is a fantastic sensor. The only issue I've ever had is that it falls off on a hot day or if I'm moving a 4,000 pound dolly at work, just because of the way my muscles and skin are.
    I've also used the Libre 2, it's just a bit larger and it works. Not bad at all.

    Like others commenting here, I have a 780G and Guardian 4 sensor closed-loop setup. I hate the sensor design. Very annoying after use having to separate transmitter, clean adhesive off it, charge it, install new sensor, remove needle (sometimes fights me), tape, insert transmitter (which also sometimes fights me), tape again, then wait for two hours and repeat in only 7 days. The first day SG readings have been erratic on more than one occasion. I've had three sensors fail in 6 months (replaced free by Minimed). SG vs BG have not been that great, but hard to eval with the lag. Previously, I've used a Dexcom G6 and it was too large and competed for abdomen space with my infusion set. I had a Libre 2 free sample my doctor let me try and it snagged on a door before it even got out of warm up. I'm going to try a Libre 3 Plus just to see how it is (and leaving the loop system for a bit). I do like the idea of closed-loop, but it has not been a panacea for me. My A1c actually went UP after the first 6 months of use compared to finger sticks! FWIW. Everybody is different, this is just my own experience.

    I have a 780g pump and Guardian 4. It isn't perfect, but works fairly well. I did try the Libre 3 for about 4 months but the software on the phone is garbage. But the idea of using a Libre 3 with the 780g would be amazing.
    I have zero interest in a tubeless pump. Reservoir has to be filled every 2 days to prevent insulin degradation due to body heat. A tubeless would be even worse. Plus tubeless pumps are horrendous looking. I also can't imagine sleeping with a tubeless.
    Anyhow, the moment I can replace my Guardian 4 with a libre 3, I am there!

    Libre2 user but currently testing Simplera, with a view of getting Inpen. Not very impressed with the sensor... I have been checking with fingerpricks when readings differ substantially...and I think Simplera is less accurate than Libre2 (despite multiple calibrations, only in the third day of use out of the seven max. it is getting better readings); in general Simplera reads low (15 mg./dl) ; and the lag is substantial. The Simplera APP is awful (I have not checked the Inpen APP yet). Not sure if adopting Inpen just because the Simplera sensor... Thank you for your great channel!

    I have had T1D for 20 years and have tried the Omnipod in the very early years (discontinued due to an ingredient in their adhesive), switched to Medtronic 722 + CGM but found the Medtronic CGM very unreliable and difficult to insert (mostly just unreliable). It got so unreliable I switched to Dexcom while still on the 722 pump like many others at that time. But it was worth it to get more accurate CGM readings. After the Medtronic warranty was up I went with the tslim so I could use it with the Dexcom. I tried a Libre CGM years ago (got a sample at the pharmacy - the Libre where you scan the sensor) and had a severe allergy to that one as well, although not nearly as bad as the Omnipod. The pods would itch and ooze after a day and it gradually got worse. My very first pump was the Deltec Cozmo in 2007 and pumps didn't have CGM back then but you could store your carb counts for common foods - much like what the Omnipod seems to have now these days.
    I just got back from TCOYD ONE and I'm hopeful for the Eversense CGM - it's not connected to any pumps just yet but hopefully soon.

    I like the idea of tubeless pumps. I do not think algorithms are as helpful as they sound. Since we know of nearly 50 reasons that blood sugars fluctuate, there is no way you can put say 10 into a pump algorithm and it work. I have heard testimonials of dozens of people that use closed loop and they don't sing the praises of it. You still have to lie to it, correct it (which leads to messing up the algorithm), turn if off to stop fighting with it, lead a very predictable lifestyle to make the algorithm somewhat accurate, etc. And the better you are at tech (like you) then the better you will be at manipulating it. When you try to correct a high, it backs off on basal. It is a constant fight. I know of some that use Control IQ and they only use that part for sleeping and turn it back off the next morning. And the 5000 reasons for pump alarms (both sound and vibrate) to interrupt sleep, movies, driving, etc. are maddening. Kind of progress and kind of not.

    Hi Justin, thanks for all information you always sharing with us .. I red about collaborating between Abbott and Medtronic, it’s really confusing they spent money and time for simplera why they just now collaborating with Abbott ? The collaboration is confusing they announced the simplera after one year will last for 10 days instead of 7 ! Maybe this collaboration will makes simplera last for 14 days later on what do you think ?

    Would you do a show about the InPen? I don’t understand why you would want one. Why can’t you record your insulin dose instead of having the pen record it. Would enjoy an in-depth review. Thanks.

    Medtronic is so proprietary. I won’t even consider a Medtronic system. The need to open up all systems to allow mixing and matching of components. The pump, algorithm, CGM and infusion set. I should be able to exchange each piece to suit my needs. If I want a 7 day wear infusion set with my Tsim I should be able to use the Medtronic infusion set. If I want to use Dexcom with a Medtronic pump with the control it algorithm it would be possible. Open devices should get priority at the FDA. Most infusion sets are made by one company. Imagine the cost savings if you only had to make one version and use it anywhere.

    I had problems with both libre 1 and 3. They both are too surface or something cause my monitoring showed zik zak line all the time. The 2 week last is awesome! Now I have medtronic 780g with guardian 4 sensors, how the f*ck you have to charge transmitter half hour and maybe it will warm up in 2 hours in 2024, I have had a lot problems with bleeding. And 1 week last i s a joke.

    Libre 2 is the only good Libre as it lets you edit notes and tests you can keep I diary almost. Nice to remember why that BS is high or low. It stays on, no rashes from adhesive.
    I Understand Medtronics, wanting to be like Apple iPhones and keep everything proprietary, diabetics already have to go through enough hell with insurance and refills and parts and pieces. Let’s work on STANDARDIZING. 🙏🏼😊

    Tubless insulinepomps is the future. The CGM disapear because the problems to have the CGM and insulinepump at the same sight is a problem that de the CGM does not work for 30-31days. Omnipod had a cartrigde for only 200 insuline units is for type diabetics who use more insuline they don 't chose this insulinepump. The most type 1-diabetics make the choice which algoritme works at my body. Is het possibele to chose make, produsize from these parts to make you one insuline which the patient is happy and don 't have to choise? We al patient make our dicesion and what is good to managen my bloodsugars. Why do insulinepump factories to chose which CGM and algorithme?

    My medtronic sensors are the biggest waste. I miss my dexcom so much. I remember leaving medtronic 10 years ago cause the sensors sucked so much. I came back to medtronic recently, I am so sad the sensors are still just as bad 10 years later. I'm dying to replace these god awful sensors. DYINGGGGG!!!

    Before I was put on the pump, I was on Libre and loved it! Such a relief from all the finger pricks! Then I was put on Medtronic 780g pump with the Guardian sensors. I thought the sensors was a JOKE! I have to charge it up for 1/2 hr, then wait for warm up for 2 hrs, then do this all over again in 7 days! That’s nuts! I’ve always wanted a Libre sensor to go with my pump and holding my breath for that! We don’t even have the Simplera here yet (Australia) and no idea when it’ll be available. So looking to you guys for your experiences as we in Australia seem to be the last country to get anything new! 🇦🇺🦘🐨

    Hi Justin, Thanks for the video. Your channel is great. I am on MDI and use Dexcom sensors myself, the G7. Is Dexcom losing touch with the market? Here in Germany I'm waiting for a discreet AID pump, not necessarily an Omnipod. The launch of the CamAPS FX algorithm and the interoperability with iOS are constantly postponed. I am losing patience. No one is willing to provide information and my supply is about to run out. Should I try something else, maybe the simplera (@medtronic)?
