Thank you for your most educated and informative detailed explanation of the all on 4 process. Unfortunately, last August I had a dentist attempt to do a few implants in my lower mouth and he drilled too deep in my jaw and severely damaged my nerve. I subsequently had nerve reconstructive surgery which I’m told will take up to five years to fully heal. In addition to this unfortunate incidence, I was sold on the 4 on 4 implants so I decided to opt for this procedure since I was desperate to have permanent teeth. Again, about 1 month ago, my dentist (different one with good reviews), started this procedure for top and bottom in the same day.I was told the procedure would last between 4-6 hours, but it was done in 21/2 hours and I was sent home with very uncomfortable dentures on the top and a bridge on the lower mouth. Since then, I am unable to wear the upper denture. It is big and clumsy and very uncomfortable. There is pain especially in the upper right, not withstanding pain where the nerve damage occurred in the lower jaw.. I went back to see my dentist 3 times and still unable to wear the temporary denture. I would appreciate any recommendation from you. I live in Florida, US and wish I had found you earlier. It might have been worth it to come there for my procedure. Thank you kindly.I

That was so informative!! I’m having my consultation tomorrow so fingers crossed I’m a good candidate! I’m so worried that my bone density won’t be enough.. I’ve read that some people have to have bone grafting done.... I have my fingers crossed that my jawbones are adequate!
I would Be interested in how much they cost here as I’ve looked in to turkey all on 4 dentures ..I’m sick of not being able to smile through my severe anxiety and my phobia of dentists … I just want to smile again …but I poor n the prices over here are extremely high
I'm guessing the actual worst part is the fact that at least in countries like the U.S. most people can't afford these things. My dentist quoted me $16,000 to do the top. Or $4,000 to have my upper teeth removed and get an upper denture. And I still need a partial for the lower. The amount of credit I was able to secure would only cover about 60% of it. And that's if I max out everything which means my credit score will take a huge hit. But it still doesn't get me the whole way.

They would fly off the and oafter website, shipping and hire some working who are usually found at home depot or lowes hardware you'd earn $5000 per week! I truly love the fact that you learn from your mistakes and admit to them! So many times a patient has walked out of an office with a terrible experience, with the Dentist blaming the patient for the faliure, when they've done exactely what's been asked of them.. I want the teeth that you did on your patient! So beautiful! Where are you located? How much is this procedure? Do they use only the mini implants or a combination of both? What if you have two regular implant in place already, can they be used in combo with the mini's? I have 2 upper implants on the side of my upper palate. I got them many years ago when I had money. But then I had to stop the work I was doing in my mouth and ended up with a denture on top. I hope I haven't overwhelmed you with questions! 😀
Hi Christine, I did this for a patient about two weeks ago, it can definitely be done, but there are a few weeks when you will hate me because the lower denture will not fit properly. If this is okay, then it's a really good solution.
Im watching this, two days after having 12 teeth removed and 4 implants set in upper jaw. No temps, get my permanent Zirconia arch bolted in in three more days. The swelling is much more intense than I anticipated. Pain is about 5-6/10. Not over the top, but not comfortable at all. Knowing what I do now, I would still have done it. Just be prepared, it has been a tough few days.
6:50 "Level the bone". This sounds horrific to me. How is this done? Saws? Planes? Files? It sounds like woodwork rather than dentistry. I've heard that up to half an inch of bone on each jaw (upper and lower) is just removed and thrown away for this procedure, to make room for the denture. Is this true?
I had "all on 4" uppers and lowers both done under sedation in one day. We started at 6am and finished with temps in place at 7:00pm. I'm a pretty tough person. I've endured a lot of medical problems and surgeries. This all day procedure was incredibly brutal and painful. The swelling and bruising, though expected was much more intense than I had anticipated. I was instructed to take Tylenol every 8 hours. It did not help at all.🤦🏽♀️
Years ago,i had two implants and was bouncing off the walls with a solute agony... mimOD.d on painkillers,mi was screaming and rolling round... never known pain like,it.mim off to sofia for my whole,bottom jaw but as inhabe no teeth there nothing should be that bad..
I had it done last Nov. 2021. It hurt me just to drink water.!! A week ago June 23,2022, they just placed me in my temporary teeth. They said I would be without teeth for about 3 months. I was with out teeth for 7 months.!!!! Then I only got 10 on uppers and 10 lowers. They said I have to wear these 1 month, then they will take them out, make any changes, send them out to the lab that did them, and they will make my new ones out of zirconium. Which means I will probably be without teeth again for a good 4 weeks! I don't see why they can't make the changes off the computer diagram and let me where my temporary teeth till I get my new ones. My procedure was in March of 2021 and I am still wearing temporary teeth. If I ever get the final product, they better be the best teeth in the world. Nothing like being lied to your face. I am post-op one week now and I am still hurting. On day 3 the real pain began. I believe it's most intense pain I've ever had, including broken leg, gallstones, Crohn's disease. This was worse then all of these.
This procedure is not painless. I had four implants put in on the top and four on the bottom on 3-30-23 it’s 4-4-23 and I’m still swollen bruised and my mouth is very sore! I cannot see how anyone can have this procedure done and feel fantastic 4 days later!
I had no pain, just soreness. I took a paracetamol the first night. That said, i found the actual procedure fkin horrific. Absolutely terrible time i had.
Hi. I had this done several years ago. I have 6 implant screws in the top of my mouth to retain a full set of the upper teeth. When I go to my dentist once a year for a cleaning he replaces the screws that hold the denture in. Each of these screws cost $150 dollars. So, a cleaning costs me $900 every year. Is this normal? Can't he just use the same screws over? Thank you.
I can't believe he didn't mention the happy gas, there is no way i am having this done without being sedated with some of that, most offer it these days, you just feel nice and floaty and don't remember much of it! Iv'e been through a lifetime of hell with my teeth, and i will be having them done in the new year, January, full set, i only have 14 teeth left, i can not wait, can't come soon enough! Think i will ask if i can just take my own painkillers, i'm used to prescription ones, don't think ibuprofen is going to cut it lol

Hello, as an older person I’ve recently visited a few implant surgeons. One has suggested using my own teeth effectively as the screws for my removable denture. Apparently this was an old way of doing things pre implant days. So they flatten 4 teeth, cover and add magnetic abutments- this sounds great, less trauma and hopefully less cost, but does it work, and have you ever used such a process or seen it done may I ask?
What happens if you have a full set of dentures but they jut don’t work I can’t eat with them I put that paste in bit they still won’t stay I need help I’m tired of being toothless I’m 64 years old

Hi, I'm sorry I do not know anyone there. I wouldn't recommend boxing. If you get hit in the mouth, you will break the teeth or lose the implants, even if you wear a mouth guard

I enjoy your videos very much. i have been in my temporary dentures since Feb 2018. my question to you is , have you ever heard of the 3 on 6 ? they are 3 seperate bridges on 6 implants with no gum line . i was wondering what your advice would be for someone who absolutely cant stand the bulky acrylic , should i do 3 on 6 or all on 4 ?
Hi, yes, you can have 3 bridges on 6 implants, and if there is bone, this is a perfectly fine way to do it. Depending on how much bone height you have lost, you may still need to replace pink gum with the bridge. I am not sure it will reduce bulk, the best thing to do here is use an implant system with very small multi unit abutments, so Nobel, Astra, Neodent, Straumann all have these small components - and these are all premium implants.
Hello Dr. Sehmi! Just curious... If in case the patient could afford it and wants it, would a dentist agree to put 32 individual implants with separate crowns each? (instead of all on four?) Or would it cause problems in the long run?
Great videos. What is the youngest age you have performed this procedure on? I am in my early 30’s and I am receiving a lot of criticism about wanting this procedure. A specialist told me that I may out live my implants. But I don’t have many other options.
Hi Amanda. I don't think I have treated anyone in their early 30's (for this all on 4). The truth is that you will be likely to outlive your implants. Then you may need to consider bone grafting and new implants (very complex) later on. It is really important to explore all of your options, and even if you think your teeth are hopeless, see several dentists who may be able to offer you good solutions. I always tell my patients that its better to hold onto your own teeth for as long as possible, and delay implant treatment as long as possible. Just coming back to the age thing, I have placed single or multiple dental implants for people in their 20s, so they would be in a similar boat, they will possibly need implant replacement later on in life.
Do you have to remove the rest of your lower teeth to get the foreign one if they are in good condition? The bottom right I’m missing all my teeth and the bottom left I have a few teeth left. So get can I get like a partial for and one for the bottom right and since I have a few teeth left on the bottom left where I had a bridge remove can I get a partialFour and one or may be at a two and one
Hello Dr, NICE VIDEO. THANKS . If a patient is treatment planned for both upper and lower implant supported denture. Is it better to do one arch at a time as you mentioned. And if both have to be done at the same time, what is the best way to approach it and what precautions or consequences should we be aware of ?
I prefer to do them on separate days, but some patients prefer to do it all at once. If both are done at once, it just means a longer time in the chair for the patient. Clinically there are some differences and getting the bite correct is often the hardest part- but this needs to be done if you are doing it all on one day, or on separate days.
Everyone should understand this is NOT the way most of us do the procedure. This is the way Malo did it in the 90s an early 2000s. We are a full digital protocol now which makes the procedure predictable and far fewer post operative complications because of the passivity of the bridge. Don't have a denture "converted" into your teeth. Find a Full Arch Master and get bleeding edge dentistry that left the analogue world behind.
Thank you! I just had this done 3 weeks ago, I'm wondering if my permanent set will be much less bulky than the one i have now and its causing a lisping my speech. There was no pain at all from the teeth and gum, the only discomfort I expierenced was from the abdominal pain from the antibiotic and not eating as much and the loss of taste for the first couple weeks . Now I can pretty much eat everything.
@@miar.4507 I am doing well and getting my permanent set in a couple weeks. I think I did get IV sedation, basiccally I was as sleep for about 5 hrs while they were working on me. Still adjusting my speech and still has a little lisp, how about you?
@@miar.4507 Im just getting regular acrylic, its just cheaper and much easier and cheaper to repair if needed to. I didn't know zirconia is heavier. Did you get yours done? If so when and how do you like it so far? Its been 8 months for me and I like it but I still feel it in my mouth and I feel like my mouth is dryer than before. I wonder if there's a point later when I don't feel it anymore.

Yesterday I went to visit this dental office in Austin Tx and the guy want 18000.thousand for the 4 All one including the zirconia. He show me the zirconia bridge and boy that was heavy. Then he say he wanted to general anesthesia. 😲 I was hoping for IV sedation just to keep you relax for 500., but he wanted do general for 3000. I'm still searching for another dentist. I just can't afford the general since is to much. So glad you getting close to the end of your treatment. Thx again for reply back.

Joanne, Please google Newme Dental in Houston TX. Dr. Beck and his staff do extremely good work. He is currently charging half off the usual cost. Complete upper and lower all on 4 or 6 if possible implants for around $23 - $26 thousand depending on your choice of material for your final teeth.
Anti inflammatory are no where near enough to quell the pain on day 2/3/4. Try an opiate based pain killer. I was stuck to the roof with pain till I took the correct pain killers. Day 5. Happy days. .
Oh yes, and I've had a green & yellow neck from anesthesia alone! Its horrible when all you've experienced all your life is horror at the hands of your Dentist! It didn't actually hurt, but it took two weeks to get back to normal.
hey dr so i have a question one of my front teeth are bigger than the other i tried making it shorter it worked but it still look weird because the sides of my teeth are different i will be getting braces soon and after i get my braces off does the dentist reshape them making them both look even or do i need another appointment or is it up to them? i don’t want to look weird smiling with both my front teeth looking uneven

With all due respect, it does hurt after. More than an ibuprofen is needed. I've gone through this, I know.

I can't even imagine the pain level. Looks like something out of a horror film. If you are wanting it done don't watch how they do it on UA-cam.

Hello, as an older person I’ve recently visited a few implant surgeons. One has suggested using my own teeth effectively as the screws for my removable denture. Apparently this was an old way of doing things pre implant days. So they flatten 4 teeth, cover and add magnetic abutments- this sounds great, less trauma and hopefully less cost, but does it work, and have you ever used such a process or seen it done may I ask?
Trust but verify . A dentist might promise you the moon and not deliver. It happens all the time.

Hi, I'm sorry I do not know anyone there. I wouldn't recommend boxing. If you get hit in the mouth, you will break the teeth or lose the implants, even if you wear a mouth guard

I enjoy your videos very much. i have been in my temporary dentures since Feb 2018. my question to you is , have you ever heard of the 3 on 6 ? they are 3 seperate bridges on 6 implants with no gum line . i was wondering what your advice would be for someone who absolutely cant stand the bulky acrylic , should i do 3 on 6 or all on 4 ?
Hi, yes, you can have 3 bridges on 6 implants, and if there is bone, this is a perfectly fine way to do it. Depending on how much bone height you have lost, you may still need to replace pink gum with the bridge. I am not sure it will reduce bulk, the best thing to do here is use an implant system with very small multi unit abutments, so Nobel, Astra, Neodent, Straumann all have these small components - and these are all premium implants.

Hello Dr. Sehmi! Just curious... If in case the patient could afford it and wants it, would a dentist agree to put 32 individual implants with separate crowns each? (instead of all on four?) Or would it cause problems in the long run?

Great videos. What is the youngest age you have performed this procedure on? I am in my early 30’s and I am receiving a lot of criticism about wanting this procedure. A specialist told me that I may out live my implants. But I don’t have many other options.
Hi Amanda. I don't think I have treated anyone in their early 30's (for this all on 4). The truth is that you will be likely to outlive your implants. Then you may need to consider bone grafting and new implants (very complex) later on.
It is really important to explore all of your options, and even if you think your teeth are hopeless, see several dentists who may be able to offer you good solutions. I always tell my patients that its better to hold onto your own teeth for as long as possible, and delay implant treatment as long as possible.
Just coming back to the age thing, I have placed single or multiple dental implants for people in their 20s, so they would be in a similar boat, they will possibly need implant replacement later on in life.

NICE VIDEO. THANKS . If a patient is treatment planned for both upper and lower implant supported denture. Is it better to do one arch at a time as you mentioned. And if both have to be done at the same time, what is the best way to approach it and what precautions or consequences should we be aware of ?
I prefer to do them on separate days, but some patients prefer to do it all at once. If both are done at once, it just means a longer time in the chair for the patient. Clinically there are some differences and getting the bite correct is often the hardest part- but this needs to be done if you are doing it all on one day, or on separate days.
Everyone should understand this is NOT the way most of us do the procedure. This is the way Malo did it in the 90s an early 2000s. We are a full digital protocol now which makes the procedure predictable and far fewer post operative complications because of the passivity of the bridge. Don't have a denture "converted" into your teeth. Find a Full Arch Master and get bleeding edge dentistry that left the analogue world behind.

Thank you! I just had this done 3 weeks ago, I'm wondering if my permanent set will be much less bulky than the one i have now and its causing a lisping my speech. There was no pain at all from the teeth and gum, the only discomfort I expierenced was from the abdominal pain from the antibiotic and not eating as much and the loss of taste for the first couple weeks . Now I can pretty much eat everything.

@@miar.4507 I am doing well and getting my permanent set in a couple weeks. I think I did get IV sedation, basiccally I was as sleep for about 5 hrs while they were working on me. Still adjusting my speech and still has a little lisp, how about you?

@@miar.4507 Im just getting regular acrylic, its just cheaper and much easier and cheaper to repair if needed to. I didn't know zirconia is heavier. Did you get yours done? If so when and how do you like it so far? Its been 8 months for me and I like it but I still feel it in my mouth and I feel like my mouth is dryer than before. I wonder if there's a point later when I don't feel it anymore.

Yesterday I went to visit this dental office in Austin Tx and the guy want 18000.thousand for the 4 All one including the zirconia. He show me the zirconia bridge and boy that was heavy. Then he say he wanted to general anesthesia. 😲 I was hoping for IV sedation just to keep you relax for 500., but he wanted do general for 3000. I'm still searching for another dentist. I just can't afford the general since is to much. So glad you getting close to the end of your treatment. Thx again for reply back.

Joanne, Please google Newme Dental in Houston TX. Dr. Beck and his staff do extremely good work. He is currently charging half off the usual cost. Complete upper and lower all on 4 or 6 if possible implants for around $23 - $26 thousand depending on your choice of material for your final teeth.

Anti inflammatory are no where near enough to quell the pain on day 2/3/4. Try an opiate based pain killer. I was stuck to the roof with pain till I took the correct pain killers. Day 5. Happy days. .
I don't know where you are, but its near impossible to get any kind of opioid anymore in the US.

Yes they can. Surprisingly the time they are in surgery is almost never an issue for them.

Oh yes, and I've had a green & yellow neck from anesthesia alone! Its horrible when all you've experienced all your life is horror at the hands of your Dentist! It didn't actually hurt, but it took two weeks to get back to normal.

hey dr so i have a question one of my front teeth are bigger than the other i tried making it shorter it worked but it still look weird because the sides of my teeth are different i will be getting braces soon and after i get my braces off does the dentist reshape them making them both look even or do i need another appointment or is it up to them? i don’t want to look weird smiling with both my front teeth looking uneven
