It's possible that high FPS is only exacerbating the issue not the root cause. For those trying the fix, make sure v-sync is on and also disable frame gen. Another possible fix that some people are suggesting is to change from 'hold' to 'toggle' in the accessibility options. I'll update if I learn anything new.
It's pretty funny. The first skills I unlocked were the 3 kills during a faceoff, and then it consistently stopped working. The fixed fullscreen seemed to fix it!

For anyone who doesn't want to change the graphics settings, I found a solution, try pressing Tab to open the map and then close it. It works for me every single time.
I actually found a permanent fix on my end with display settings maxed out. Even dlss super res on quality and low latency on. Update firmware for gamepad if using one, then initiate a standoff then pause and reload from a checkpoint. It works every time after that.

DLSS for AA. 120Hz monitor, unlocked frames at around 70 to 100fps. approximately 10 to 8Ms frametimes. I will try to lock it at 100Hz with V-Sync switched on in game and the Nividia Control Panel. @@Gillium I've just tested two long sessions. Both at 59.94Hz. No bugs at all with Stand Off so far. I put my 120Hz TV to the in menu setting of 59.94Hz in Windows 11 and I turned on V-Sync in the Nvidia Control Panel. I also have it in Exclusive Fullscreen and G-Sync is on. I think V-Sync is on in the game menu as well. No issues so far! Even with Boss's/ Mongol Leaders after the cutscene. This game plays well at 60fps so I'm not concerned about increasing it. Thanks for the tip!
Setting refresh to 144Hz and vsync to half refresh rate (and then limiting to 70 via Rivatunner to eliminate all possible input lag) did the trick here (EDIT: the glitch still happened, even if much rarer, so had to go bring it down to 60fps after all). Also set "other actions" to toggle, so the prompt doesn't clutter the screen and it actually activates with left click instead of awkwardly holding strong attack.

I have different steps, 100 or 80 fps and this bug still happens on 100 fps and I don't want to play on 80.. I did not buy a strong pc and a 165 hz monitor to play on 80 fps ffs (I know I can also cap it in nvidia control panel) So they did not even test this game with high fps at all? like wtf
In my case, holding right mouse works, but pressing left mouse button doesn't in standoffs Also, special abilities like heavenly strike never works (v + left mouse)

I don't know if this is a 100% foolproof fix, but waiting like 1-2 seconds before holding down the button works for me. I'd wait for the enemy to approach then hold the button. If I do it as soon as I see it it sometimes bugs and not register.

When I was testing, I found that the in-game refresh rate limit doesn't even work without also enabling v-sync.
Incredibly frustrating.

Works for me so far, much better fix than capping the framerate.
I'd rather set hold buttons to tap in the accessibility page instead of lowering my fps.

You need to spam any of the buttons or keys on your keyboard

I tried the toggle suggestion and it works properly more often but it's still very inconsistent.

I fixed it, I put it on Exclusive Fullscreen, but instead of decreasing the FPS I went nuts out of defiance and tried 240hz. Works like a charm
