I know it seems like a simple concept but I feel sometimes it is difficult to explain the pathophysiology especially in the case of desaturations rather than apnea because one would think their bodies would react with compensatory tachycardia but instead their HR lowers. Is their brain simply that sensitive to desaturations that it causes an immediate parasympathetic response? I have watched so many of your videos and I so greatly appreciate you sharing your knowledge and insight!

Honestly- we don't really use the neonatal GCS that often. it's very hard proving a baby is brain dead- but we do have some criteria. Otherwise, we often use the HIE Sarnat scoring or evaluating more subjectively. You're right though- this isn't ideal!
We have no control over which ads are shown where. To use the fantastic platform of UA-cam means we have little say in many things. So obviously many people are watching ads that are not relevant to them etc. thank you for putting up with them because that’s how it allows us to continue! And yes! A video on transfusion guidelines is a good idea!
I don’t know how to send a direct message to you. Would you be able to do education on PICC lines in the NICU? Please! Is it necessary to measure the extremity of insertion? Our PICC lines have a warning to not use syringes smaller than 5ml. My management says that it’s ok because the medication is going through an extension set, and it is also ok when y’ing into carrier fluid. Is this true?
Hi Natalie! Sorry took time! Our direct email is talatalksnicu@gmail.con We def need to do more PICC line videos! A lot of PICC line usage stuff is hospital dependent. If a smaller syringe is used then obviously it can give more pressure to the line - so obviously you wouldn’t want to push anything with a smaller syringe but your hospital may allow it on a pump (if can be done?). I will try to find someone who can give a lecture better than me!
Thanks, do we have to depend on urine output in the first day? Coz, Some neonates won’t pass urine in the first 24 hours. And is there a specific timeframe after initiating treatment to consider the absence of urine output a concern? 🙏
You’re absolutely right! I don’t really consider UOP that first day - just want them to urinate once. Having said that - if infant on lots of fluids- eg a gastroschisis baby or a 23 week baby- I would like to see more than one wet diaper!

No we haven't done one. I've used NAVA- but not recently- and so I don't feel like I'd be best person to discuss (even though I love it! But all the little clinical changes- I don't have that down!) I'll try to find someone who will!
