First of all, THANK YOU dr mellick for continuing to post videos especially during these times :) you and your team are appreciated! One quick question: how often does the maintenance guy get called into the ED for tool requests and consults on how to remove objects and what not? Someone should get their department an award!
Larry Mellick i‘m a doctor too and i think i would have done the same! But i really think it‘s only for the feeling and have no benefit! Greetings from Germany
@@WilliamTGM Yes, I have driven the autobahn a number occasions between Stuttgart and Frankfurt. We would usually fly out of Frankfurt. Great memories!
It would be more painful injecting lidocaine bc it would sting and wouldn’t be as fast. As a kid who’s been through it all, I know first hand😅 using the spray distracts the patient as well as slightly numbing it
Kids are wimps. They get a papercut and scream bloody murder. Some wouldn't be able to handle the needle. When I had awake surgery on my toes, I screamed bloody murder because it hurts so bad.
should have shown the kid how to do this cause if he's gonna grow up fishing he will do this more than once..i have pulled them out of friends scalps..
I remember I was a little boy my brother and I went fishing at Edgewater park (Lake Erie). My father had forbidden us to go, but we snuck out and went anyways. I got a fish hooks stuck in my thumb. We went to the snack bar and asked for help. Who was there fixing there refrigeration unit? My father or course. He removed the fish hook and I paid at home later.
What a brave little fella... but I'd never let those nasty rusty cutter pliers near my child... sterilised or not... buy that poor maintenance man some new tools...
I love it when maintenance gets involved
interdisciplinary medicine at its finest, love it!
Glad you enjoyed it!

It is not uncommon. However, once we get our own Dremel and side cutters, it will be much less common.
Bless him! He did so good! I know plenty of adults who would squawk a lot more than that!
LOL i love the celebration music
Thank you, Dr Mellick
Antibiotic prophylaxis or just monitor and start if necessary?
Just monitored and will start if necessary.
Thanks for posting
Thanks Mellick for sharing experience .. is it more easy to remove it forward rather than backward ?
would you do an X-ray prior to removal to determine if bones have been affected?
You definitely could.
That’s a huge fish hook!
I use had a huge hook in my feet. It hurt like hell. I pull out myself
Ouch! I don't even know if shoes would have prevented that giant hook from going through! He was so brave!
Sidecutters sterilized?! Why? Was the hook sterilized before it punches the boys foot?! 😂
Hah, good question and practical. It's probably the "optics" and makes everyone feel better, but the side cutters never touched the patient.

@@WilliamTGM Greetings! Early in my medical career and with the US Army, I spent 3 wonderful years in Stuttgart. Absolutely love Germany.
Larry Mellick i‘m an anesthetist at Frankfurt university hospital. It‘s only about 120 miles to Stuttgart! Keep up your excellent work!

Thank you very much.
That fish hook was VERY VERY huge.
I am feeling doubtful is it enough only using lidocaine spray?

@@paytonsflavor8322 I agree.

Just came back from a and e with a fishhook in the side of my foot just like this one.
what are the types of immunisation given to such patients ?
read a book
A tetanus booster if needed, plus verbal and written instructions for signs and symptoms of infection.
If the barb wasn’t through, would you advance it first? then cut it?
Yes, that commonly works. There are other techniques. Check out my other video. ua-cam.com/video/YyymouV8ulU/v-deo.html

Lidocaine spray?
If so.. localized lidocaine could habe worked too right or is it not recommended in these types of injuries?
(Localized lidocaine injection*)
Freezing spray to numb for a second they said injection would be worse pain then spray
its over so fast, dont even need pain killers. i would have removed it myself, not every operation needs to be 100% pain free
The injection would hurt worse than the spray
Might be beneficial to keep a couple of (sterile) wire cutters in the ER for situations such as this.

KUDOS!!!
That poor little guy
Ouch! Ouch! Poor little guy.
I caught a boot but there was still a foot in it.
Did you take it to the ER?
Seems that way.
I bought an old civil war boot from Sol Rosenberg that still had a foot in it.
Ouch! This is def cringe worthy. Tough kiddo though!
always wear sneakers!
😱😱😱
Did the maintenance guy just pull that out
No that was an emergency medicine resident.
Ronald Rump 😁
Larry Mellick I wasn’t sure had to ask 😂
@@lmellick is it wrong if a maintenance guy pulls it out?
@@PoetryPickle uh yes? They aren't qualified. They work on buildings and stuff. Not flesh

It says that it was sterilised before used.
🤦🏻♂️
rust isnt even that dangerous
Oh Debra... id be more worried about the fish hook IN THE FOOT causing Tetanus than pliers lmao
