I appreciate that you´ve made a video to help inform people, but this particular information is the information that comes up on the first page whenever you google the question. What I can´t find is how to use a user name and password for your proxy. Or I should say I can´t find one that actually has information that works.

Adding a proxy username and password is not supported on a Windows computer. You would be better off taking that off the proxy.
