I've called numerous times, and ask for "payment tracing" agent, EVERY single time its a busy line, so its near IMPOSSIBLE to talk to them first.

Thanks for this video. I was wondering what the top part was about. I received a refund check, but it was made out to my deceased wife's first name, middle initial, and nothing more. It's impossible to contact the IRS by phone these days.
You have not told me how to find my IRS office in California where to send the comleted form to.
As well as you have not addressed the fact as what to do if No refunds were received 😢

Hi! I have question on section I question #7 about refund amount, are they asking only for federal tax return or do i add up my state tax return? If not can I also use this form 3911 to request for my state tax return? Meaning i will have 2 forms to send to the irs am I right?

I called 100 tines to IRS never got an agent. I signed as single can I fill out the form?
If you are a single filing taxpayer, you can complete Form 3911 and sign by yourself. It is only in cases where taxpayers filed a joint tax return (Married Filing Jointly) where both taxpayers would need to sign.

Hi! Fantastic video. Thank you. I live abroad and lost my first stimulus check (EIP1). I’m preparing the 3911 form to fax in. If it came as a check, can I ask for direct deposit? I remember the first check was for $1400, but in the list it says $1200 is the maximum payment for the first stimulus check. Should I leave the amount blank online 7, or should I write the actual amount that was on the check (1400)? Thank you again! 😀

Thank you for video. Question, Can you send this form 3911 if it was for a business? It was for a ERTC Credit and we did receive a copy of the cashed check from IRS. It was stolen and cashed. Been back and forth with IRS. They said to call the Bureau of Fiscal Service to file a claim. We did call and the Bureau said that the IRS has to put a code on our file.

No, you should file a separate Form 3911 for each check.

i never recieved my third impact payment, did the rebate instead of this on my taxes and got error, is sending this going to fix this? any help?
The EIP3 payment should first be claimed on the 2021 tax return if you never received the payment. If you claimed the payment and the IRS adjusted your tax return because they believe you already received the payment, you should first call the IRS to speak with a representative and start a payment trace. Then file the Form 3911.

What if you want to receive it direct deposit?
You can enter direct deposit details on the phone, but we recommend the paper treasury checks as they seem to be more reliable when it comes to requesting another check
