In Canadian documentation, the "Given Name(s)" field is where you include both the first name and any middle names. Since "Bin" is part of his name, it should go in the "Given Name(s)" field along with "Hussain." The "Surname" field is reserved for the family name, which in this case is "Usman.

yes, you need to have known the guarantor for the duration that is mentioned in the instructions
