    I wish there was a more simplified list of the types of procedues available and the benefits and costs to each type.

      basically there is 3 different things with a couple of folow up options as far as i know.
      1. Vaginoplasty (with varying depth)
      2. Vulvoplasty (0 depth)
      3. Orchiectomy
      i think these are the basic three options for mtf srs

      @oliviawutam hii, I'm mtf too and I'm pre op and am thinking I would like 0 depth. I have 2 questions, how was the recovery, and are you able to... you know... If I can still "feel good" post op I would get the operation without a second thought.

    Dr. a young lady was born with a zero depth years ago and a Dr. was able to have her sit on an English saddle with a dialator at the correct angle who would increase the depth after many sessions! Have you heard of this before?

      jimmie little yes - this is true - it seems to work for vaginal agenesis patients but not transfeminine patients that we know of.

      I have to travel frequently for my job. I would be more than willing to come to Miami Florida. Should you might have a opening at your clinic. I would have to require at least a month notification from you. So as that I can work my schedule at work. Thank you so very much for taking the time to respond back to me. I have been in contact with other doctors and after seeing your videos I am 100 percent sure that you and your team would be my best choice for the SRS bottom surgery. Thanks Sharon

    Hi, I don't know if you'll see this. But I have been thinking about wanting this surgery for quite a long time now, but one of the things scaring me off is that I have a blood platelet disorder. So my blood takes a long time to clot and stop bleeding, I can't have any type of blood thinners and various medications. I have heard in the past that having major surgeries could be a potentially big problem, so I'm just wondering if this would rule out the possibility of me having a surgery such as this?

      The best answer comes from your prospective SRS surgeon.
      Correspond with as many surgeons as you can. Present your specific condition to them. Provide any applicable medical records for them to review, to discuss with your home medical team.
      I have read from many Trans women who had serious medical histories and they were easily accepted for surgery because the correct surgeon had the expertise to perform the surgery with the necessary adjustments.

    I think the zero (minimal) depth procedure would be best for me. I believe my insurance will cover it, but it's questionable whether they'll cover travel and hotel and such. I looked at your website and feel confident you'd be a great choice. Can you tell me what state your practice will be in? I live in New Mexico. Thank you for your videos, they're the best that I've found.

    Hair removal and possible hair growth inside vaginal canal are major factors for me to choose zero depth. 😶 But i'm not sure if i can get full depth via colovaginoplasty in the future.

      Electrolysis removes any hair that might be on skin used to create your vagina.
      Some SRS surgeons do Follicle Cauterisation during surgery. This procedure has been done since the 1970s.
      Also, Estrogen itself will greatly reduce or eliminate any hair.

    Dr. Gallagher, I have a question. In MTF Trans women do the remove the prostate or can it be removed. Do trans women still need their prostate? I'm interested in eventually getting bottom surgery with a full vaginal canal but I need to know what do they remove and what they leave. I'm only 3 months into my transition so I have a long time before I can get it, I just am doing my research. Thanks, Dr. G.

    I'm curious if they can do it with any depth at all because I'm being told that I can't get a full vaginoplasty because of concerns of forming embolisms. But that really limits my sex life with my girlfriend. I mean can't I get a couple inches? The procedures not that different from vaginoplasty.

      partial is possible, consult your surgeon about the possibilities.
      Nobody but your medical team will be able to give you anything more than theoretical options after all.

    Full-depth surgery is not a lifetime of dilation. That is a misconception.
    Dilation is important during the first six months to one year post-op - some surgeons schedule it as much as three hours per day.
    Once you complete your requirements, dilation becomes more an option - actually becomes desirable as masturbation.
    Also, your Gynecologist can easily feel your prostate through your vagina.

      She never said it's a requirement for the rest of your life.
      But for many people it can be problematic because of their circumstances.
      For example if you have a full time job, you'd be pretty much incapable of performing that job for several years (which is how long I've heard from multiple sources you need to dilate) because it'd be rather impractical to say the least to interrupt your work every couple of hours for half an hour of dilation (especially when you can't work from home, but even if).
      And yes, the clinic I'm on the waiting list for mentions 3 hours a day as typical for several years, the amount possibly reducing over time.

      * Is the Trans woman working 24 hours per day? Dilation can be done in the morning when you first wake up and another session before you go to sleep. Or you can use flexible dilators to do dilation while sleeping. Thus, plenty of time to take care of business during your first six months post-op. Beyond that first six months to a year - YMMV - you can decrease your amount of dilation time and number of sessions, no work time interruptions. I know of no SRS surgeon who requires dilation ‘every couple hours’. *

      A man has no need for a gynecologist, and gynecologists aren’t trained to treat or screen prostates. See a proctologist or a urologist.

    This is a big decision. Even more so because of the need for hair removal. I don't even know if I can dedicate so much time and resources to this stuff at all. If I could push a button and have woman parts I would, but go through all this? Idfk.

      it is a major decision, and a massive time investment.
      Hospital I'll be using (closest to where I live that is) mentions 9 months to a year for the hair removal, then 2-3 months at least to recover from the surgery, then 3 years of dilating several times a day.
      Mind that that hair removal treatment would be an hour or two every other week or so (not counting travel time to the skin clinic performing it), so can be done on an afternoon off from work for example.
      It's after all a major, very intrusive, procedure. Which is why it's good that zero-depth is available as not everyone can undergo such major procedures with acceptable (medically, personal, or whichever reason) risk levels.

      Some surgeons do Follicle Cauterisation during surgery. No Electrolysis required. Estrogen will greatly reduce or eliminate hair there after a few years.
      Dilation is not necessarily that extensive. Usually, a few hours per day for the first six months, lesser time as you continue. You can do your Dilation while you sleep. You can wear a flexible dilator while you are doing your daily activities.
