    This video has given me the answer I need, I have seen this video and its given me a solution to what I'm looking for, I'm a transwoman and lesbian who doesn't seek penetrative sex at all due to some unfortunate and unclosed childhood trauma. The prospect of having the external features of a vagina with a fully functional clitoris sounds like the perfect pathway for me.

    This is really interesting. I had never even heard of this before. I've been researching everything regarding depth not 0 depth. I personally think the positives out weigh the risks for getting the depth included. SRS or GRS is such a personal thing that only you can decide what's right for yourself. Here in Canada I have to be on HRT for 1 year and then they will cover the cost of my SRS or GRS. So I'm definitely going to be getting it done, the more depth the better, as it was only the money that would have prevented me from getting the surgery. I'm actually on day 3 of HRT!! I'm so thirsty for water all the time lol. Great information as always. You're one of the good ones. Thanks Doc!! 🤗

      Because of my age 60 and having a minor stroke even in a lesbian relationship it seems the less risky option 5 years ago I would have had a vaginplasty

      I agree. If I am going to go forward, the more depth the better. Maybe peritoneal pull thru or whatever. I saw zero depth on pornhub, and I saw partial limited penetration in copulation also on pornhub . Now I live in Texas and I cannot change my legal deadname. But I am working out how to go forward. Thanks for sharing. Right now this is my only community.

    I'm currently recovering from zero/shallow depth vaginoplasty at age 27, and my only worry has been getting it at all - but I know that's pretty common in the early healing process. I'm certain I'll feel a lot better once i've healed and can return to "normal" life. My reasoning was that I've had a perfectly enjoyable sex life without my genitals for nearly a decade now, so why take a massive risk on something I might not even enjoy? I don't know what having a natal vagina feels like and I never will, so I didn't want to bother with dilation. Hoping I'll feel happy and confident with the results within a few months!       Thank you for sharing and I wish you a speedy recovery. Sexuality is adaptable and we can always learn new ways to be in touch with our bodies. Having a vaginal canal is not a priority for many and thats ok.

    That is great advice for young people. I also agree that older trans women should also keep an open mind. I am over 60 and have only transition little over a year ago. I was married for 36 years and have no interest in that type of sex and don't want to take the risk and work that is required to have a canal but do want to have the zero depth surgery.

    Im new to the channel, i came to this video after seeing an Instagram post from the lovely tattooed Doctor John Brosious of VPSI saying he was going to do a vaginoplasty and vulvaplasty in the same day, and didn't know what that second one was.

    So, if you already like sex via number 2, then this option can be less risky and an viable option for you. And i think this option is omitted in the discussion. Toys, penetration from males, etc., all still available via zero-depth procedure, provided you just want to keep using the hole number 2 as you have been up to now anyway. I'm getting the zero-depth procedure in 2 weeks.

      @Grace Hardy that's very very unfortunate. The choice to have zero depth seldom has anything to do with one sexual preference. The ignorant presumption that only heterosexual women prefer penetrative intercourse is incredibly outdated. I am sorry this happened to you.

    I’m 24 and scheduled to have this procedure in April. I have a orchiectomy now and I got it to prep myself for srs, but I don’t think I would want to use my lady part for intercourse but anal yes. Who knows I might cancel it’s a huge decision to make. But I’d always said if I were to go thru with the surgery I’d be glad it’s 0 depth the other one extremely too much work for something I have to do all my life.

    I'm 50 and not active, and I've been thinking of zero-depth (I understand they call it labiaplasty in the UK), and I told my two psychologist consultants that that's what I wanted. I'm having my first meeting on Wednesday with my surgeon consultant, and I'm now leaning more towards the full vaginoplasty.
    I'd like to avoid dilation in my lifestyle, but I certainly perceive that I could experience regret if I don't go all the way. And I don't want to go through the rest of my life thinking what could have been.
    From what you're saying, regret is not a hypothetical thing; it can happen down the line. I'm also enjoying excellent health. Right now I'm at the point where I'm agreeing with you, and I should go all the way. It might be the only way to put my mind at rest, even if it ends up never being used.

    Hi. First thanks for sharing you knowledge. I’m considering a zero depth. I’m a 56 year old. I’ve been medically transitioning for almost a year. I am married to a cis-gender woman who is 11 years older then me. At this point my libido is non existent. I’m really ok with that. My wife and I are extremely close and have a non existent sex life. After more then 22 years of marriage we still grow closer every day. She is slowly excepting the changes I’m going through. I feel so lucky to have her in my life.

      @Phil Sylvester thank you for sharing. Please keep in mind that your sexual desire may heighten once you do get bottom surgery due to overall congruency you will feel. Of course, that is not a guarantee and you should still do what feels right. I have had clients get zero depth and never regret it as well. For those who have considered all the factors, it is a viable great option.

      Very similar situation 56 years old married to my wife 2 years younger since 26 years. We don't have any sexual activity for years, which is not a problem. The poor libido, I have, vanished with the oestrogen. In the past during few sexual intercourses, I have, I always want to be penetrated, rather than penetrating. I'm not planning anything for the time being as I have a pretty good life and a SRS of whatever kind will not change my world.

    Hi Dr Z , I have exchanged coments with you before and you have been extremely helpfull , when you percieved that I was all in my head about coming out I dug so deep I found that the reasons for not being able to express are because I am autistic , as soon as i found out things started making sence and the world paradaime shifted into me seeing it through a difrent filter which has empowered me to come out

    I am 29 pushing 30. I am not sure if its just a combination of HRT, dysphoria, and other things, but I am beginning to lean more towards being more and more asexual. I still have plenty of time to reconsider, but I've been thinking for a while that I want to choose a zero depth vaginoplasty. Just told my boyfriend who is also trans that I am leaning more towards asexualuality. I just don't get much out of sex, I'd rather just eat tacos 😂 Dilating seems like a massive pain, and the healing process takes considerably longer. I'm still conflicted, but I have plenty of time to figure things out. About the only argument I can come up with is: Would I regret it if I went zero depth?

    Hi!! Thank you for the video :) I'm very early in medical transition, only 3 months in HRT, so this intervention is still in the far future. But I've been thinking about it anyways, and it's really difficult to decide. I'm 25 years old, but the dilations, the complications and the fact that I'm asexual make me more interested in the zero depth. But as you said you never know what live will bring in the future. Oh well, I have a 5+ years of wait list to decide.

      I’m happy you’re giving it thought. Sexuality can be more fluid than people realize, especially if you haven’t had the right hormones all your life. Even as an asexual it’s hard to justify going the nuclear option on my next 50+ years of life

    I was considering getting a zero depth surgery, but my surgeon convinced me - with the same reasoning you use in this video - to get a vaginoplasty instead.
    Dilating is an ordeal every day, it gives me so much distress, I hate it, it hurts, it bleeds... my neovagina is not at all usable for the "intercourse" everyone keeps bringing up, even 6 months after surgery. I can barely fit a finger or the smallest dilator in there, let alone a penis or a toy!
    At this point I'm considering stopping dilating, but I'm not sure how that would affect things. Can you advise on that? Would it just close and end up as if I got zero depth, or can I have health issues from it later?

    I'm 52 and I have two chronic conditions which I'm told rule out vaginoplasty. I would love vulvaplasty but due to the prejudice against it in my nation's health care system, I can't even get a zero depth vagina.

    Oh no girl, it'll be full depth for me should I get bottom surgery and I probably will. I'm still at the beginning of my transition and I still have so many other things to work on, like electrolysis for example, that my current genitalia doesn't really bother me in any way. But even if I never have sexual intercourse again I think getting bottom surgery and only having a zero-depth neo vagina would cause me some form of body dysmorphia.

    I resonate with the idea of preserving sexual function. I am older and don’t have and negative response to my male organs thanks to dating pan women who celebrated everything. This is why I am not taking hormones or messing with my junk. As long as I get my face and chest in order, I’m good. It was already a horrendous catastrophe to have my foreskin stolen from me as a baby, robbing me of those sexy nerve endings...would not risk loosing the orgasm pathways I have become accustomed to. Everyone’s mileage varies, but that’s my vibe and I’m only attracted to women. I’m in my forties though and I just began my official transition last week, though decades of life experience count as part of the big picture transition journey for me. Dr. Z. You are an amazing resource. Stumbling upon your channel has saved me probably months or years of research. Your insights mixed with my regular therapist (whom I will be coming out to in two days) are all I needed.

    Hello Dr. I have chosen to do the zero-depth vaginoplasty. My decision has to do with my age, being sexually assaulted by men 3x, I'm going to be a trans lesbian & I have my own massage practice, so I like the quicker recovery & not have to dilate. The only bad part is, I'm a US retired military veteran, so the VA & Tricare won't cover our "bottom" surgery. I'd luv to hear your suggestions Dr. Thanks so much. ~ Sophia : )

    I belive that a vulvaplasty is my best option. I am 62 yo, and have pretty much been asexual all my life. I do, however have "issues" with viewing the mirror in my bathroom, as I get out of the shower, and the fitment of my clothes. We all have our own goals, and put empasis on different parts of transition. Be you, and be happy!

    Very interesting.
    I too tend to be asexual and now that I’m older I still think that I would like to retain the ability to have sex. Aging In addition to my intersex body type type have always presented sexual issues that my wife and I have had to deal with our whole married life together. Even though I tend to be asexual I do enjoy our relations that we can preform. The gift of loving bonding between two women seems as natural as anything else in my world and would work for us very easily.
    She has come to enjoy time with me as Hannah and accepts me as a women when we are together. The issue is I really have built my whole life around my spouse and would find trying to live as a women with any other person in a relationship imposable. In many ways I see this as being unhealthy yet there is a real gift in having such a bond. Very complex and confusing and yet wonderful.

    I just set my date for a PPT in July. Funny thing though, at the beginning of my transition, I didn't want anything. Then that didn't feel right. So I moved to the possibility of an orci. I knew by now the boys needed to go. But after a while, that didn't feel right either. I then felt the zero-depth was right but now, I feel the need to go with the PPT. I could save myself a lot of pain, hassle, and money with the zero, but now, I feel that I would simply do it for the wrong reasons. To just make it easier on myself. Even though I now feel the need to have the depth to be fully myself.

    Hi Dr. Z! I am older and have never even considered zero-depth vaginoplasty. The first time I heard of them, my reaction was why? For me, a full depth vaginoplasty is not primarily about sex - It's about getting as close to whole as I can after being inflicted with the wrong body. The truth is that I would much rather risk dying on an operating table trying to be whole than being alive and fall far short of what is possible - I want it all - I am entitled to the right body and, even though I'll never have it, I can get close.     I'm considering vulvoplasty, due to my age (65), the relative lower cost of that procedure, and the faster recovery time. Re intercourse, if there's a vag. "dimple", a man can slide between one's thighs for a fairly pleasant feel, and, of course, there's always an. interc. Your thoughts, Dr. Z?

    i would get a Orchiectomy i think but in reality im hopeing one day in the future ill be able to get a vaginal transplant but I'm not comfortable with anything till then

      Peritoneal pull through vaginaplasty could be an option for you, it's kinda experimental, considered riskier and not yet widely available but in a few years there will be more data and ways of reducing the risk of complications.

    Hi, I've been thinking of which depth GRS I wish to proceed with. My question has been partially answered. I have been on Hrat consistently the thought you broguht about "Whether or not we would like/desire pebetrative, toy pleasure and/or males. Thank you!
    The remaining question is zero depth and half depth regarding S/O, I lean more towards females, reiterating sexual desires, is orgasm without penetration still attainable?

    Dr. Z, I get UA-cam feeds through Amazon Fire Stick on my television and an algorithm, I think mistakenly, often sends me anti-transgender videos which I fear are muddying the waters of the discussion. Detransitioning themes seem to be popular from the cisgender world of late, and perhaps those posts are waning. Actually, most detransitioners don’t discount the experiences of the trans community and realize their reasons for transitions were atypical and not well thought out. Today, however, I was sucked down the rabbit hole of a group of people who identify their experiences with the concept of autogynephilia. I was under the impression that this sort of diagnosis of transgender ideology was long-since abandoned by modern psychiatry. But these people believe this concept and that they are victims of this affliction. Are they not self-loathing trans people or psychotic individuals who are not transgender at all?

    I can see that surgery being useful for older patients like you say or people really sure that they don't want any kind of sex . I'm curious though , would a surgeon be able to create a canal later if the person changed their minds? Or maybe it's not do able because there's no scrotal skin ?

      It would be very difficult and risky because its a major reconstruction of a reconstruction using colon graft.

      @Kylie Tanner that's a great question. I did speak to a surgeon about this and he said its possible but depends on various factors including finding a harvest cite such as colon lining etc to create a canal. However, he said its not as easy and may not work for everyone.

      @@willowZzzzzz Not only is it a colonectomy, its a graft into an all ready traumatized area full of scar tissue and ad-hoc blood vessels and lymphatic drainage.
      I'm sure its possible, but the risk of potentially fatal complications should outweigh the ability to play "hide the wienner". IMO =+)

      @@obsidianjane4413 What are you talking about? I had this conversation with Chett a couple years ago when I was over there. They just simply part the cavity tissue like normal, and line it with grafted skin (like normal). The problem is donor skin (ask Jazz Jennings). We do these kinds of grafts and repairs all the time. Colon was an experiment in depth for a while, that turns out to be not worth the cost (for most people).
      Everyone: Go talk to your surgeon.

    I am 50, and largely asexual, and don't want to bother with dilation, so zero-depth seems like the right option for me. However, I still have some niggling doubts about zero-depth, that maybe I will regret it later.
    But I am not in a hurry about it. Next year I am going to get an orchidectomy. That actually seems more important to me at the moment. After that I will decide what I want to do.

    Hello mam i am very very confused due to a question related hormonal replacement therapy and fitness and the question is that if a man wants to become a female by hrt transition and he is totally into bodybuilding,weight lifting ,strength training doing exercise and workouts and activities like that if he will be able to these activities before,during and after the transition ????,will bodybuilding,heavy weight lifting,strength training,cardio,mma, effect the transition in a bad way or not ???and plz also try to explain my question into a vice-versa way for female who wants to be a male. 🙏🙏🙏🙏🙏🙏Thank you

    I’m not quite sure what I want to do. I don’t like the healing process for the neovagina. It smells and you can’t have sex in the same position or even have sex with well endowed (and girthy) partners anyway. I feel like the smell of the constantly healing hole would ruin my sex drive because I’d be insecure. I’m not trying to sound judgmental this is just my fear after speaking to multiple post op women.

    I know the full procedure is challenging, and I know everyone comes with their own baggage, but... I personally can't see a way I wouldn't want to get as close as possible to my gender identity. I had never heard of this option before, and it rings alarms bells with me.

      There are people who are asexual by nature or have been sexually traumatized where the requirement, complication risk, and ongoing maintenance requirements of it aren't worth it to them.

      I'm not criticizing. That is just my perspective and I'm only 50. I've only got 20 yrs. or so left in this flawed body, I can put up with it for a while longer and help my kids and others.

    Could you go into more depth about this procedure. [Ouch! Excuse the pun]. I'm 76 and just starting HRT. I don't have any idea how far I might want to go. No partner at present, and none on the horizon, but who knows? She would be female, I feel quite sure. I'm interested in what/where/how would sexual functioning work for someone like moi?

    If I had this surgery I would probably go zero depth or shallow depth. I don't have all that much penile tissue being very small and sex has never really interested me.

    It's important to discuss with people their options and go over the pros and cons of the different procedures. You state this well in saying that patients should never let someone else make the decision for them. However, while you may not say directly that people shouldn't get zero-depth, you heavily imply that it isn't a favorable option.
    Multiple times in this video you state that you don't see this as a good option and that many end up regretting it, or that even though this may clearly be the decision the patient wants, it isn't the best because of a hypothetical "what if you end up regretting it;" something that lgbt+ and especially trans people hear far too often.
    If someone knows that they aren't interested in penetrative sex or sex in general, it isn't your place to say they should opt for a more complicated surgery to preserve a sexual function that they do not want and may or may not ever use. This also ignores that while everyone has their preferences, vaginal pentrative sex is far from the only option for pleasure or intercourse.
    You also state that zero-depth is a procedure done primarily only with older patients or people with sexual trauma. This simply isn't true, and can be harmful as, again, many lgbt+ people are told that their sexualities/sexual preferences are wrong and caused by trauma.
    Is your goal to tell or urge people not to choose certain procedures, or to provide information so that they can make healthy informed decisions on their own?
    I'm surprised you're making these suggestions as someone in your field, especially to such a broad audience. Simply because you don't see/understand why someone wouldn't want to have a neovagina doesn't mean it isn't a good or favorable option for that person.
    Please I'm know that I am truly not trying to attack you over this. I just want to offer you another perspective and show how your words can be interpreted. I'm very thankful that you are willing to put up informational videos on sexuality subjects, but the tone of the video and your phrasing are very important and put this procedure in a specific negative light.
    I'd like to hear your thoughts and potentially have a meaningful conversation about this topic. Thank you!

      Hi and thank you for commenting. I can see how one can perceive my views on this surgery as negative, by no means what was intended. To be honest, perhaps I find it challenging to speak about this procedure from a too neutral stance only because I offer free evaluations for gender surgery, as a result, I have seen people who expressed either regretting or seeking a letter for a revision. This of course, in no way means the procedure is wrong or bad, and in hindsight maybe the "negativity" you perceived could be what I have developed internally as a result of feeling empathy for the people I saw who expressed how they felt. That's why I encourage and urge people to think about their own needs and always make decisions that are best for them as my opinions wont always be right for everyone.       I have to agree severely on a lot of what Misteph has posted. There was a lot of pretty hurtful things that you said in your video, espeically for those of us who are older Transwomen. Your words have pretty much written us off for having a sex life because of our age when you said that you would not recommend it for younger Transwomen. That was so mean.

    I would like to explore the option of there is a option of full of transplantation of the full female reproductive system including the replacement of the Vulva, vagina cavity, cervix, uterus. falliopian tubes & ovaries. That is what I would really like to have done.

      To my knowledge this is not available currently. There are some experimental studies on cis populations, but granted given that it is cis population, it is either to transplant one organ when some or other elements are already present. As far as know, this is not even a possibility for trans women at this point.

    Speaking as a older trans woman(57) who's started on the road to transition I have thought a lot about vaginoplasty and have been doing research on it. I even talked to my insurance company (they don't cover it 😢). I never head of zero depth until now. While the idea sounded good at the beginning of the video after hearing you won't have any type of penetration I know it's not for me. While I may never get a partner I like the idea know that I can still enjoy penetration via other means. I learned something very valuable today! Thanks Dr. Z!!

    Probably the same kind of health care provider that would refuse to tie a woman's tubes "without her husband's permission" or until she's had a specific number of kids. Or refuse to do a vasectomy on a man until they were 45.
    I'm trans, I'll do my own research, make my own choice, and I'll pass on your advice... Maybe keep your "personal opinion" out of my life altering health care. Just because you think everyone needs the opportunity to be a buck wild sex machine doesn't make that the case... I just want to pass and be able to wear clothes without fear. Thanks.

    Вы абсолютно правы уважаемый доктор. Очень часто сексуальность человека претерпевает во время перехода глубокие изменения. Причем как у тех мтф кто предпочитает женщин, так и тех кто предпочитает мужчин, да и те кто вообще считали себя асексуальными тоже просыпается многое внутри. Я бы выделила 3 группы ложных предпосылок. Те кто предпочитали изначально мужчин разумеется занимаются анальным сексом и почему то думают что будут делать это и дальше. Те кто предпочитают женщин почему то думают что не переключатся на мужчин и им будет хватать стимуляции клитора. Но практика показывает что большинство изначально ориентированных на женщин тс в России все равно переключаются на мужчин. Асексуальные же тс часто не понимают что первопричина их асексуальности кроется в не принятии собственного внешнего вида и с удивлением обнаруживают что обретя желаемое полное внешнее соответствие женщине у них еще и либидо прорезалось.

    could they keep penis intact but hide it ,over pubic boen and then up under belly skin , or into the body cavity , or make a hole for it to stick it into when want to hide it, using a skin graft maybe, but where to hide the balls or just remove them guess most dont want the testosterone , , maybe loosen its roots to move it back then hole under bone , but disconnect urethra, l never seen any such ideas discussed like if not possible why,o
