I.D. My Door Part- S4001 dual adjustment Stamped on hinge Pat Nr. 5.701.636. Replacement is S4003 and S4004 hinge.
S4003 with the vertical adjustment Set Hinge and S4004 for the Horizontal adjustment Guide Hinge. Some door manufacturers only use the s4004 on all 3 hinges spots others used both s4003 and s4004 - where top and bottom hinge get S4004. And the middle hinge get S4003.
