why do you have combat mode set to the L2 button? I certainly don't recommend it because you are an experienced player, but if you reassign it to, for example, square (dualshock), then you have a convenient button freed up, for example, for guardbreak or dodging/cancelling an attack. Also, you don't have to constantly hold it down
It's a bit weird, same thing happened to me before, if a heavy lands at the same time with your heal, the execution kills you instantly, there are also some cases where a player executes a person but that person leaves at the start of the execution and another person gets executed instead of that person no matter what his hp is.
