I missed and exit and made a wrong turn in Chicago, once, and came across a low clearance. Luckily, I had room to pull to the side, and I made a phone call and waited for about half an hour for police to show up and hold up traffic so I could pull into a side street then back up enough to turn around and head the other direction. We have a low bridge where I live. It has signs warning you of the height, it has height sensors that trickers some flashing lights if you're to high, and it has some reflective plates hanging down in chains some 30 feet before the bridge, so if you're gonna hit the bridge, you're gonna hit the plates first and be warned before you slams into the bridge.
