Edit: my mom later explained the characters were mentally immature to understand the concept of sex and relationships etc. Some examples are Em freaking out about her period. She had no idea what was happening to her and (obviously) freaked out. Another is Richard freaking out the moment Em gives birth to their baby. He had no idea what was going on and panicked. Same with their sexual urges. They were unaware of what they were and simply acted on them, not knowing the consequences.
