If its a reflection of the real meaning or not, upon listening to it for the first time, my best friend who has schizophrenia had some interesting comments where he felt like he could relate to the lyrics:
Intentionally or not, this song well describes a not uncommon perspective of a person with schizophrenia or bipolar or another presentation of psychosis who has undergone treatment. The 'agony and irony' of being deemed in recovery from a mental illness and yet finding their life now has much less meaning than it did before the illness:
What may later be diagnosed as psychosis may be scary, confusing for some but for others in addition to this it may be very exciting and give life a new meaning- as my other friend with bipolar would say "when you're on the part of your illness that is a good trip" vs the terrifying and unbearable parts of the "bad trip" aspect, it doesn't seem like an illness by its nature but it also feels like you've broken through into something greater.
From the perspective of someone going through something like this: after sectioning, after hospital, (maybe forced) medicating and being told by the psychiatrist and nurses who hold power over your life etc that you were out of your mind- ill- but now they are making you better, your freedom depends on taking these meds (with all the debilitating side effects), taking piss tests to make sure you are off the 'bad' drugs that they don't want you to take. With that in mind you might feel like you were better, more alive beforehand vs feeling cut off and disabled (ie an amputee) by sectioning, meds and being forced into the role of a patient. Above all bored and frustrated as fuck with this new normality-' particularly if you thought you were doing something important, yet nobody believes that but yourself. You feel alienated from your loves ones, even betrayed if they helped get you sectioned.
So from this perspective for this person in a manner of speaking the 'cure' may feel worse than the disease that they are told they have- and whether that reflects reality or nor it is a really strong feeling and not without grounds- the lack of freedom and decision over life that comes with sectioning can be horrifying, and that's merely by design and ignoring any staff who abuse their power.
For some people meds help a lot with minimal side effects but also might just sedate- to make people less 'troublesome'- but with awful, embarrassing, disabling, even dangerous side effects, yet with no effect on the psychosis itself- just the ability of the person to function well enough to express themselves. For most people on meds it's in-between two extremes. There are even meds for side effects with their own side effects. It's one reason why a lot of people struggle to comply with taking these meds even if it's a term of their freedom.
Some of the lyrics seem like 'world salad', a term describing a way of speaking common to some presentations of psychosis; the words have meaning to the speaker but to others seem strange and alien with odd word choices- sometimes with practise they can be deciphered by someone who is familiar with the person- but in extreme cases it seems like utter nonsense to the listener- frustrating to the speaker.
Even though voices can be terrifying and even like having your own internal daily bully, there can be a flipside where they can be interesting company and in some cases seem to have their own personality and biography. So to lose them or lose their potency in treatment can actually mean that some people feel sad or even grieve for them.
"I run it up the flagpole and see who salutes but no one ever does" sounds like a word salad way that my ex (who also has schizophrenia- we all met in the secondary mh services years back) would describe meds making it difficult for him to get hard, about him not being able to wank; erectile dysfunction and related problems are a common side affect of many antipsychotics and sometimes it even persists after they've been discontinued.
Some of what you said is true, and I am speaking as a person with bipolar disorder who has paranoid delusions.
For the most part the stuff about mental illness in the song is correct, but the way we treat mental illness is one aspect of the song.
The song is about alienation. The words are not word salad. They all have specific reasons for being there. The artist has told us what run it up the flagpole means. Its a masturbation reference. There's also a reference to recreational drug use.
The song is about trying to figure out how to possibly fit in, how to exist in a modern society thats moving at 100 million miles per hour every day and how the regular person gets dissolusioned and left behind.

May sound like its not alienation, but listen carefully. He feels like he wants to fit in and do things that cool people do.
Even the parts about you told them all I was crazy and the cutting off the legs are about how all of those experiences you described in your comment lead to feelings of alienation from society. The song is about the artists struggles with mental health, but its part of a larger issue with society and the people around him. Even the drugs and cant forget the curves of your body leading to the flagpole reference are all about escapism which is like the peanut butter to alienation's jelly.
