Thank you for this podcast. I am an EMT and am regularly called on to move people who are multiples of a healthy weight. 500 pounds is not out of the ordinary. I can’t “give medical advice “ but I want to be able to share some knowledge and promote the belief that something can be done.

I love a new interview with Dr. Bikman, but I am really bothered by how the last part of the interview turned into an ad for the hosts exogenous ketones. It's a conflict that should be stated clearly.

Why are we discussing the law of thermodynamics in this conversation? That’s for a closed system. We are an open system.
Hence why the "waste" needs to be accounted for. The Law of Thermodynamics still applies.
So a T1D has no insulin resistance if they don't take the insulin their body needs to stay at normal blood glucose levels)? If they took just enough to circumvent DKA, there's no insulin resistance because their insulin levels are low? Even if they needed boat loads to bring glucose down to normal levels?
T1D is insulin dependent. T2D is insulin resistance. T1D insulin receptors are working properly where T2D is not.

A T1D is not immune to insulin resistance. A T1D can have malfunctioning insulin receptors just like a T2D can.
Why are we calling carbohydrates and fats calories? A calorie is a measurement of heat and by the packaging can be off by 20% so you really can’t even track a calorie. If you eat 200 cal of steak and I eat 200 cal of bread they are not gonna act like a calorie in our bodies. They are going to act either with an insulin response or not. And that is where that storage comes in to play.
No. Protein takes more calories to digest. That is why you can eat more calories of protein than carbs or fat and not gain weight. It has nothing to do with insulin.

Kind of. You are referring to the thermic effect of food which is actually quite small in the overall CICO equation but nonetheless this Insulin insanity must be stopped. You only gain weight by being in a calorie surplus .. it’s physics. All the biology comes afterwards at a micro mechanism level and while important has been used by these charlatans to sell useless products

If ketones is the preferable fuel for the brain, why then the body still keeps a stable level of glucose in the blood (through gluconeogenesis) in the absence of carbs in the diet?
Ketones is not the preferable food for the brain. If you have no glucose you will pass out, go into a coma, and eventually die.
brain is not the only tissue in body. Some cells need glucose like red blood cells.
This point shows that in the absence of carbs, the body will produce what's needed to operate properly. Try living without protein or fat and see how the body reacts. This confirms what we need and don't need
I was taught that you were born with a set number of fat cells and these could increase or shrink in size throughout your lifetime but that you never increased the number of fat cells in your body - is this not true ?
