- Thinking about getting fillers? It's important to keep a few things in mind before choosing to get fillers. Dr. Karam shares how fillers have contributed to a common phenomenon called pillow face. Pillow face is the result of consistently getting fillers in order to replace lost volume in the face. The issue, though, is that filler never truly goes away, and overtime the face becomes over-filled. We see many celebrities, like Madonna and Courteney Cox, who look completely different now because they've repeatedly used fillers. Dr. Karam prefers to replace lost volume through the use of fat transfer, which results in a more natural look that lasts longer and keeps you looking like yourself.

Dr Karam can you please make a video on fat transfer without the face lift? When is this the right option and what results can you get? Can you get the same results as you would with the filler and what is the actual difference in what filler can do as oppose to fat? Can it be injected into lips the same way as filler - there are different technics depending on the shape that you want. Thank you

@la_reine_en_moi Why would a physician NOT do procedures on people of color? What do you think would be different? I do understand that question for facelifts and procedures with possibility of keloids/scarring-but race should not an issue for fat transfers etc. I am confused about your question. I know many Black and Latina women and others who have had many procedures. If you have not had your first procedure and have some hesitations, say that.

The irony is that fillers are touted as non surgical like it’s a good thing ; when in actuality surgery is whats needed to fix the problem and still look natural.
I haven’t had fillers and I find the look very off putting. Artificial looking.
I can't undertsand some celebrities. They have the money but they don't know HOW and where to spend it when it comes to fillers and surgery.
Improper guidance and we didn’t know much about fillers before. Also when you fill your face that becomes your new normal your baseline, so you don’t see yourself becoming overfilled. Your used to what you get each treatment. That’s why it’s important to know your baseline and have before photos and always reference them to make sure your not going to far from what volume you initially had. Or your ideal look/ treatment you initially got. Filling for anti aging purposes means you should refer to your pics when you were younger, but if you want to enhance natural features you should be aware of when you first achieved your ideal look. Then try to maintain that and refer back to it so you don’t see yourself get overfilled and think you need more each treatment when you need less. Patient advocacy is super important. A lot of injectors themselves do end up looking kind of overfilled.

Unfortunately transfer of fat in face is very fast metabolized, it's gone in no time.
I have no volume loss in the face and I look great! :) When I see videos lile these, it makes me feel even better about myself! :)

I wish I can show my picture and show what procedure I have done.
With all his education, yall still asking stupid questions. He said no fillers but fat transfer looks more natural. Fillers don't help with sagging but with loss fat.
I didnt get a facelift because its not affordable but I do my own research so I don't look weird and still like me.
48= look 35. I'm also petite, do strength training 4 times a week. 5'1" 105 lbs: 32,25,34...My shape and tone tight body already makes me look younger.
Here's what I've done
Botox, fillers on my peripheral cheeks not frontal when I was 44, still there and natural, I used one syringe.
Fillers on lower eyes and nasafolds, but again not the whole syringe. A little bit...
Lip lift (the best surgery by far with my breast augmentation at 35, still looks firm and natural. Went from a big b to a C because I only wanted my boobs to look round especially on top it deflates as you get older but with implants they stay round, dont care about sagging boobs, I care more of shape.)
Fillers on lips. Lastly, which I didnt want to do, but its the only thing that will lift but more affordable, PDO thread lifts. That helped my jowling. Im not gonna say oh I look good and dont have jowling. Trust me late 39s and up you got sagging. Been told I look younger than my 30 something gf by other women, not men who wanna bang.

Fillers don't lift but add volume only face lifts will fix the sagging.

Yes. They will not fully go away and often they do migrate.

So many stories when fat transfer lasts 2 months only and it’s all gone….
That is why you need to do your research on your doctor! It should be permanent if performed correctly.

I’ve been thinking about Juvederm I appreciate your advice and I don’t want to end up like some of the stars that have crazy cheekbones.😱😳😅. We take a very small amount of fat from either the outer thighs or lower stomach. It is performed under local anesthesia not general.

Please can you suggest me What to do. I turned 30 this year, and did cheekfillers last year. But now my cheeks looking bigger and i have a shadow under my eyes wich makes me look tired and older. I also started to use hyaluronic acid, i thought this could help but now i dont know if it made it worse.
