Some exceedences of the IOW(Change in corrosion rate/Feed, Pressure, temperature)? Neglect prior inspection? Some condition in the base metal (inclusion, residual tension, etc) that became enviroment sensitive?
Yes, this vessel is within the scope of the API 510. I have not had the opportunity to inspect this vessel from the inside, and as a result cannot see if there was a damage mechanism internally. No visible damage mechanism was present upon the outside surface, though the through wall corrosion may have obliterated its evidence. I was more able to review the operating conditions of this vessel to determine if that was a factor. MrEric
This vessel is used intermittently. Which option gets done is up to the client. It is a coded pressure vessel so it would need an R stamp repair or will stay out of service. MrEric

There are no indications of external corrosion.

so would that be a take the unit out of service and replace kind of job or just send the welder to patch it up?
