I think the Transmission Chain will be trying to jump off the Small sprocket on the engine clutch. The big and small sprockets are at fixed centers, the chain isn’t adjustable. If the chain is worn it will try and jump. If I were you I would remove the transmission chain case (top half) and take a look while it’s turning. A new chain may fix it

Spigot bearing maybe ?? Nor holding the input shaft in true alignment...... ??
or bearing fail. They need to take the cover off and inspect the engine drive
To old for a dual mass flywheel or add blue fault
