It is not first time, My5tCrimson added Doom assets on Sally.Exe AND Sonic.exe. The game over screen one is from the end of episode 2 "Shores of Hell". In original Sonic.exe the game over screen was from Doom 1 after episode 3 "Inferno" where X (Sonic.exe real name) with the dead bodies of Tails, Knuckles and Robotnik, until the picture changed in version 666. Also hard to notice Knuckles' fist attack sound was also taken from Revenant's fist swing sound. If I forgot something let me know.
About Cream's death: I saw a comment on a seperate video of gameplay that made this more shocking, saying that the background blurs and fades away (2:28), as if Cream didn't die immediately and the imagery is her vision slowly fading to black as she's unable to move and slowly succumbing from trauma or blood loss.

yeah it's an exe game, but i still have a soft spot for this. it's definitely one of the better creepypasta games based on the dreadful vibe it gives off
Creams pain was over within a few seconds, sally was literally just awaiting her fate as all the walls were closing in on her, a slow and torturing death
this actually is a bit better executed than other exe games from its time, even though it still has cheap jumpscares and over the top edge, it does succeed in giving off some creepy vibes at and some of the kills can be kinda unsettling too, even tho most of the creepy comes from stolen assets :p I’m probs just biased cos this was my fav sonic creepypasta game when the whole exe thing was at its height

I think this needs a reboot actually and I found the best roles for my reboot of this .exe game. Amy but her classic version, Sonia replaced with Cream, and Manic replaced with Rouge the Bat.
