  • Platelet-rich plasma (PRP) injections are a revolutionary treatment for knee osteoarthritis. PRP, derived from a patient's own blood, involves concentrating platelets and growth factors and injecting them into the affected area. This method has proven highly effective in reducing pain and improving function in various conditions, especially knee osteoarthritis. Recent studies, including one with over 3,100 participants, demonstrate PRP's superiority over other treatments like corticosteroids and hyaluronic acid in improving symptoms and potentially slowing the progression of arthritis. PRP works by leveraging the body's natural healing mechanisms, reducing inflammation, and preserving healthy cartilage, as evidenced by significant decreases in inflammatory markers.

Great video. Thank you, I now its lots of work.
I had three PRP injection within 9 months about 3 months apart in my "bone on bone" hip. Before I was barely able to walk, barely able to bend over, severe pain on rising. 5 weeks after the third injection. I am almost back to normal. It is incredible. It takes weeks to feel the effects. Sometimes months. I plan to get 3 to 4 PRP's per year indefinitely. I had several prolozone injections (10), they did NOTHING.

I tore my knee meniscus bad at 41. I did PRP without insurance and it costs 2k a few years ago. The recovery took years to get back to long distance hiking and full flexibility. I'm a Triple Crowner, look it up. I cant say whether it was PRP or just time that fixed it but it is fixed and I can run and hike when I thought never again. Physical therapy is HUGE. Use it or loose it. The night after injections SUCKED with intense pain. If I had to do it all over, I would. Thought I would never do another thru hike again and since I have hiked a 120 mile, 300 mile, 365 mile, and a tough mountainous 800 miler. My knee wasn't fully healed until 2 years and about 1600 miles of long distance hiking. That last 800 miles on the Arizona trail is what did it. At the end of that hike, it was like I was never hurt. On that hike I did a rim to rim of the Grand canyon with that knee, no problems. The road to recovery was LONG.

If it is such a proven success, then when will med insurance help out for payments? Better results are less cost to the insurance companies and to their clients. How can we pressure the to approve?

Hi Dr. Peng - thank you for another informative video. At the end of this video you mentioned an upcoming video that would cover what makes a PRP treatment successful. Will this video include information on platelet count and how to get the correct higher account, what anesthetic not to use, etc... I did watch your more likely video on PRP injections which covered some of this. While PRP treatments are available in my area. So far none of the providers have any idea what the platelet count should be and they all want to administer lidocaine which according to the other video would interfere with the PRPs effectiveness. Thanks Tim

8-10 billion platelets seems to be the ballpark # for large joints like knees and hips. Ropivacaine doesn't really affect platelets. But to make sure, I use ropivacaine to help numb down to the joint capsule. Once I got through the capsule, no more anesthetic, just PRP into the joint.

If new injections are needed after 1 year, what are the results of studies for length of time for further injections? How long can you continue injections? Or is knee replacement the next step?

Works great for many of my patients, about 80% of people who get it seem to notice tremendous improvements. The people who see less dramatic results are those with the most severe osteoarthritis. For those with mild to moderate osteoarthritis, 1 high dose PRP injections lasts around 1 year.

My 3 hip, two knee, one shoulder PRP injection resulted in PRP specimens that where translucent amber color. The Doctor told me the looked perfect. I take many supplements, I am sure that helps my blood chemistry. They all worked well.
From a different doctor, I had one in my SI joint, and one in my hip bursa sack, and they had more of a red/ orange color. I have read that this is better for soft tissue as opposed to the clear amber.
Can you tell us your opinion?
    Thank you.

The color of PRP will vary based on the type of cells included. Joints and soft tissues require different types of cells and will make the PRP slightly different color

In my experience, hyaluronic acid injections work great for about 70% of people, but the other 30% doesn't seem to do much. Doesn't really matter which brand you use.

I have a degenerative lateral meniscus, and last week my doctor gave me a PRP, but he didn't use ultrasound guidance. Do you think this will reduce the efficacy? It was injected just above the knee. He felt it would reach the necessary area, but due to the expense/research I've done, I'm nervous it may not work as well.

Studies consistently show 1 high dose PRP injection can provide around 1 year of symptom relief in those with mild to moderate osteoarthritis. The goal is to help reduce pain and symptoms so that people can work on other foundational treatments such as diet, exercise, and weight. The reality is that osteoarthritis is not (currently) reversible. And 1 year of symptom improvement is pretty darn good compared to other nonsurgical options.
