Oversimplifying it, but it's racquet acceleration (speed) and angle of racquet upon contact (spin). The pros are strong and skilled enough that it doesn't matter which area of the racquet they hit it (except the frame of course), it'll almost always come back fast and with more spin in comparison to the average player, though that is just him warming up.
I think there are several reasons. One is that pro players have developed a natural kinetic chain in their strokes, so power and spin comes with it even though not hitting in the sweetspot. Another reason is the racket these players use: they are heavier than regular rackets. This means more mass behind the ball, therefore more power.

after watching a Federer hitting practice video, watching this guy's forehand is almost literally painful! lot of cognitive dissonance trying to figure out how those mechanics add up to make a good tennis stroke. his backhand mechanics are easier on the eyes, ironically, considering his BH is notoriously bad.

all pros hit with a continental grip on there first serve but some may tend to shift slightly toward an eastern backhand grip

his forehand looks fine to me
It might not look as smooth as Roger's forehand, but he hits it with quite a bit more spin and speed.
Painful? This is as simple motion as FH can get for his power & consistancy. Best coaching comming from Italy now.
