Update June 2021:
Within the scope of the measures taken against the Covid-19 pandemic, the following decisions are effective on June 1, 2021 regarding entry to Turkey, per the Turkish authorities:
Passengers arriving to Turkey from Afghanistan, Bangladesh, Brazil, South Africa, India, Nepal, Pakistan, Sri Lanka, and passengers who have been in these countries in the last 14 days, must submit a negative PCR test taken no more than 72 hours before arrival and must be quarantined for 14 days in locations to be determined by the Governorships. Flight restrictions for Brazil and South Africa are removed.
Passengers arriving to Turkey from the United Kingdom, Iran, Egypt and Singapore must present a negative PCR test result taken in the last 72 hours prior to arrival to Turkey.
Passengers from countries other than the above, who submitted a document issued by the relevant country's official authorities showing that they had been vaccinated at least 14 days before their arrival in Turkey and/or that they had recovered from the disease in the last six months, are not requested to submit a PCR test and quarantine; and passengers who cannot present a certificate of vaccination or documents indicating that they have had/recovered from the virus are required to submit the result of a negative PCR test or rapid antigen test (performed within the last 48 hours) in the last 72 hours.
In addition, passengers, excluding those arriving from countries listed in Article 2, are subject to PCR testing on the basis of sampling at arrival. These passengers will be allowed to proceed to their final destination after the test samples are taken. If the test results are positive, they will be treated in accordance with the COVID-19 guidelines of the Ministry of Health.

I'm lucky I got to visit the country for two weeks, before they locked it down again. That was truly a once in a lifetime opportunity, to experience one of the 6th most visited nation on earth almost tourist free. I know under normal circumstances, I probably would have been disappointed with most of the places, but having them almost entirely to yourself is a completely different experience. It was really cool, because it felt like the country was just being discovered. Kind of how travel was back in the day before we had Google Earth and all those travel shows.

same here! So I should wait 72 hours prior to my departure, right? It's even easier because I can even fill out the form from outside of Turkey since I would only be there for transit and I would want to use the public transportation to reach places in Istanbul

I was just reading that you don’t need a visa or a PCR test if you’re coming from the USA to Turkey. I’m getting misinformation. I was supposed to come there in the beginning of April and was relieved I didn’t have to go through all the extra hassle.

1. I will arrive in Turkey on Aug 6th with Turkish Airlines and would have a long layover (14 hours or more transit, time) so I decided to go out with a group of people. Should I fill out this form?
2. If yes, how to provide the address information? We would now our transit hotel later on when arriving in Istanbul, however I really want to keep it simple to avoid any hassle and fill out the form from outside of Turkey. Is it possible to put address "hotel provided by Turkish airlines?"

Within :) specially PCR test, it must be taken within the 72 hrs prior your departure.

Hello! Sure! You can so that, too. Just keep that paper with you at all times and whenever a place asks for it, you can show the paper and they’ll scan the QR code which is for the HES code.

No, just if you exit the airport, but connecting passengers don’t need it.
