Both died at the same time basically. The fire extinguisher wasn't discovered until later on after it jammed a persons car door shut in the parking area. Wikipedia details the crash very well and also states that the two marshalls who ran out onto the track to help another driver who was on the side did not have proper clearance to go out onto the track at the time either. These kills are hilarious. They award you for things that make no sense... Like pushing a guy off a roof while people are watching Car racing?? dude how does that make it look like an Accident... Theres gonna be that one guy who'll see you up on a roof. Some are funny some are actually pretty cool Although this game's an okay for me.. Its not bad but its not the best.
Note that nobody sees you push Robert Knox. There aren't any guards there (You have to take out the guard before the kill) and all the spectators see is a car crash, caused by someone falling down onto the track.
They think it is an accident at first but pretty sure they realize it is not an accident after a police report. But by the time they do that, 47 is already out of country. Also, it is best game in its genre for now.
They should have a multiplayer and co-op mode, where in multiplayer you have to assassinate a real player and/or you have to race others to assassinate the given target.

Yeah, some of them are terrible ideas if you plan on getting away with it. Take the combat android kill or the electrocution kill, for instance: multiple people witnessed both kills up close. There's no way you'd walk out of either room without appearing suspicious/raising alarm, even if everyone present initially believed they were accidents. The fan one, too - you were standing at the fan controls when he got blown off and there are multiple people around.

At the speeds those things go, they are basically designed to be upside down planes. Wings and design are to keep the vehicle on the ground. Otherwise it will end up flying.
