@@eyebleached I'm not sure what the 3rd page is that you're referring to. If you are referring to the 2 copies of the 433D, send Part 1 which states IRS Copy on the bottom, to the IRS.

I have had an agreement for a few years and this year when filing online, the monthly amount wasnt accepted. Said i need to send a revision of this form. However ive never used bank for payment by direct and havent had issues. My qurstion is now on this form, do i select i am unable to make debit payments or leave that and routing area blank? Kinda confused lol. I dont use a bank account so not sure, i always manually pay via pay1040 via debit. Any help would be greatly appreciated!
Either way, you would leave the routing and account number blank. It states that if you are unable to make debit payments, check the box. If you are able to make debit payments but choosing not to, don't check the box. Hope that helps.
That depends... You can mail it to the address on the notice you've received but I would not recommend mailing it. You can request one online - www.irs.gov/payments/online-payment-agreement-application. Assuming you have no specific Revenue Officer assigned, you can contact the IRS Automated Collections System (ACS) for Individuals at 800-829-7650 and they will assist you. You will need to fax it to them while on the line. If you're not able to do that, they can help you get it started. Hope this helps.
@@bobjablonskytaxreliefhi, I’ve called that number but it’s so hard to get through. I’ve been calling for days and either I’m on hold for 1 hour then it drops when it said waiting time is between 30 minutes to 1 hour. You don’t have the fax number by any chance do you? Will help a lot

People who do what I do spend a lot of time on hold so we tend to address multiple client issues on each call. As I mentioned above, you should be able to respond via mail as instructed on the notice or try setting up a payment plan online. www.irs.gov/payments/online-payment-agreement-application

Is there an error on 433-D PDF? Seems like there's multiple copies. Do we nee to fill both out like shown in example?
Good question. Part 1 states "IRS Copy" on the bottom and Part 2 states "Taxpayer Copy". Send the IRS Part 1.
