How to Fix the Dreaded Lateral Shift - Side Glide in Standing for Low Back Pain & Sciatica

  • Dr. Sam Schroetke of Physical Therapy & Hand Clinic of Hillsboro demonstrates How to Fix the Dreaded Lateral Shift - Side Glide in Standing. The lateral shift is a condition that is often very painful and can take a lot of time to correct. So be patient and if your lateral shift is stubborn, perform this exercise in sets of 10-15.
    If you have a lateral shift, to dosing for Side-Glide in Standing is 10-15 reps every 1-2 hours or anytime you feel yourself becoming shifted again.
    If you don't have a lateral shift, the dosing for this exercise is 10-15 reps every 2-3 hours.

    I am a senior citizen who got up out of bed badly a few Fridays ago, resulting in terrible back pains, confined to one side only. I rested for the day only to discover later that i was leaning to one side. I was scared. I rested again the Saturday, however on the Sunday of the same weekend, I was scrolling on my phone and found this page. To God be the glory. After doing the exercises a few times throughout the day, by evening I was back to Normal. Thank you so very much.

    I’ve got a large herniation to L4/L5 and have the shift to the left, which strangely is the side the herniation is compressing the nerve root. This video has literally made me cry - everything he says is me!!!! I can’t do flexion or extension and every PT exercise exacerbates the sciatica in my left leg. I’ve been suffering for 2 months and started imagining never standing up again. I’ve just tried this once and the improvement with just one set is unbelievable. Thank you so much I will be doing this every 1-2 hours. You are WONDERFUL.

      I had the same thing and ended up with surgery. It thankfully hasn’t returned and I feel completely normal again.

      This exercise is for a specific sub-set of people. It tends to help most people who have a lateral shift. Unfortunately, some people do not respond. But the non-responders often become responders, it seems, after a cortizone injection helps damper that acute inflammation. Thank you for commenting ~Sam S.

    I‘m a physiotherapist and i treat patients with McKenzie all the time. If there is a lateral shift other treatments just do not work! You got to get rid of the shift first! And that‘s the way to do it! Very well explained - thumbs up for this video! 👍🏼

      @@donkeyfacepunch good question, thank you. The shift occors mainly due to the body trying to avoid pain. Therefor the pelvis is shifted in one direction to give the disc more room. Then the muscles tighten up and fixating the pelvis in its position - you got yourself a shift. Sometimes thr muscles are able to relax again, sometimes they need ‚directions‘ from the outside.

    I’ve been shifted for about three days now this is excruciating. I have ruptured L4 and L5 in the past and made a full recovery with no surgery but sometimes when I get away from my core exercises, I will begin the shift. This exercise is actually helping although I’ve tried it many times through the years and it never worked before. Lord God have mercy on all of us with this horrible back pain!

    Thank you. my nephew sent me this video six weeks ago. It took me three hours to get out of bed and do this the first time. That's the kind of pain I was in. Foot drop, numbness, blinding pain. The whole siatica package and this exercise saved me. Thank you for this video.

    I had the apparently rare case when I was tilted to the right and the pain was on the same side. Do not be discouraged, try this exercise nevertheless. I can't believe how efficient this was for me, literally felt better after 10 reps. Thank you from the bottom of my heart for this wonderful video.

      I have the same thing where the pain is the same side and the tilt and it hurts like hell. I’m scared I’m gonna make it worse since this case is more rare. Did it hurt a ton for you as well? Specifically the sliding motion back out

      I had the same thing. Insanely painful. I ended up having surgery and it hasn’t returned.

    I cannot believe how powerful this exercise is... My husband is currently suffering from this problem. After watching this video, he started to do the exercise. In the beginning, it was too painful but as you say to move on, he did not stop. After completing approximately 200 rep with multiple breaks, his posture is fixed! Thank you so much! Incredible!

    I stood up from my dining chair and was not able to walk. My back was shifted I was scared, went straight to my bed. I googled my symptom and this was the first video to pop up. As soon as I did this exercise I heard a crack on my back and I was able to walk straight. AMAZING!!!!!!! Thank you!

    Thank you very much. I have been dealing with back pain for two years and always wondered why my back was in pain. I finally looked in a mirror and noticed I had a lateral shift. A few days trying this and I've gotten it completely straightened out.

      More than a year? If you want to a Dr. he would've prescribed physical therapy. The PT would have taught this on day one.

    I can't believe this actually worked! The other day i injured my lower back and my back was crooked, until i tried this and now my back is straight. So much easier to walk now that my pelvis isn't tilted anymore. Thanks so much for this! I will continue doing this every hour

    Spent time in therapy, had a shot in the butt nerve to relax the joint then an adjustment, stretches for months with little results. After watching this video and 1 day of following your instructions I'm 80% better. Hurt like hell the first stretch but got better as I went along. Well worth it. Thank you very very much!     You should have a few million views and likes dude. (Should I say “mate”? 😂). I am a marathon runner, thin individual, light weight and pretty much very healthy but I have suffered these terrible and painful lateral shifts for many years. (Very shocking to see on the mirror actually)
    It used to take usually weeks to recover until I saw this fantastic video like two years ago. It takes me two days now! Amazing!!! You are a master and I thank you very very much!!!

    Thank you!! I've shifted in the exact position you are illustrating in the video at least a dozen times in my life and I've had to just ride it out for four to five days before I could walk upright again. It's a miserable experience. Now I have this video to use as a tool to hopefully correct the issue if it ever happens again (hopefully not!). This video popped up on Google when researching Spondylolisthesis and what I can do to counteract or correct a current unnerving issue where I can feel my lower spine move around freely. I'm glad I found your channel!

    My physical therapist instructed me in this specific exercise. I try to do several repetitions every hour throughout the day. It is a great tool

    Wow I've had this happening to me off and on for years and I never knew what it was!!... Years ago, at first, I thought it was my back, years later I thought it might be my kidney!... and then my hip flexors!.. Thanks to you and your video, I have now discovered it is a lateral shift because of a bulging disc!?!? Thank you very much! You're a godsend!

      Dana, have you been able to successfully treat yourself when you have a lateral shift? Thank you for commenting. If you continue to have this problem and are unable to self-manage (correct the shift on your own), I would recommend finding a McKenzie physical therapist in your area. The great thing about the McKenzie system, aside from being effective, is that it is all about self-management.

    Hi doc. I just threw my back out again today for my 42 birthday. The same exact way that you illustrate in your video. Could you please provide some insight as to why this lateral shift happens? What are we "pulling" or "dislocating"? I've done this so many times in my life and have received several different explanations. You're the only one that has provided a proper corrective procedure, so I would love to hear your point of view. I'm relatively thin, am athletically built, but not overweight, and lift weights regularly, in case those factors matter. Thanks again and take care!

      I don't think anyone really knows why it happens. This is a Mckenzie technique, and they believe that within the joint complex, some sort of obstruction to motion occurs. So for a lateral shift, the obstruction is located on the lateral side of one of the lubmar joints and a persons body will tend to shift away from the location of the obstruction, cuasing a lateral shift in the case.
      YOu might try doing an exercise cauled Extension in Lying, which I show in anothe video. Do the Extension in Lying only once you are no longer shifted. You might it to prevent an onset of back pain by doing 10 reps of it per day. ~Sam S.

      @@sams.4388 Thank you again for your time and explanation! I'll try that (again). I attempted a daily stretching regime meant to help with overall flexibility with a focus on the lumbar area that included that exercise. That only lasted a couple weeks though as I'm about as flexible as a pencil and couldn't do a single stretching exercise properly, which meant there was a fair amount of pain and discomfort. I guess I'll just have to suck it up and stick with it as I really don't want this to develope into a more serious problem as I get older.

    Your instructions for this exercise are the best I’ve found! I’m not sure if my body can be fixed because I’ve been getting more and more crooked over the past year and a half, or more. I’ve been to a surgeon, and he sent me to a physical therapist. I became I’ll during the time I was going to PT so I missed a couple of weeks. I d shown my therapist how crooked my body is, and he started me on some exercises, but neither he, or his staff seemed all that interested in helping me.

    I've been like this for almost 2 weeks now. Haven't been able to do any of the other exercises I've seen due to pain. I just did one set of this with about 8 to 10 reps. My butt area (kind of on the side) definitely hurts more BUT the pins and needs in my foot and down my leg are better! So far the most improvement I've seen in the last 13 days. I'm actually super motivated finally! I've been really down not being able to do anything. I will keep up this exercise!!

    I've tried the exercises he has mentioned in the video. They are excellent and have noticed improvement in posture immediately. All I need to do moving forward is maintain the routine.

    Thank you for the detailed explanation of this exercise. And the additional exercise after you do the first . My chiropractor just half told me how to do it as he was rushing me out of his office. I’m having very intense low back pain that radiates across the base of my spike , radiating up the spine and down the legs n buttocks . And I’m super crooked. Hope this helps me . Oh how I’d love to be out of this pain . Not many videos address the fact that it hurts some people to tuck their tailbone and out knees to the chest . And those are always the exercises suggested by many doctors, therapists…etc . I’m subscribing now . You’re great at making things clear .

    I’ve watched at least 3 other videos on how to do the wall shift and tried each time. They did not help. The details that he gives here, especially regarding placing your foot against the wall before you depart and only moving back to neutral are key! After his video I am literally almost completely straight. This is after only 3 minutes.. and I have had the issue for about three days. Thank you!!!

    I’m a builder in the UK and this has happened to me 3 times severely. The first time I called 111 and they said take pills and rest,I was out of work for 2 week wondering how I was going to make money and would I ever work again it was frightening. The second time it happened I was so worried and had spent hundreds trying to get better so I thought I’ll go on UA-cam and I followed this video and it worked and the third time it happened I had to find this video again follow the instructions and it worked. I walked into the lounge like a cripple and came out like an athlete. Unbelievable. Thank you for this video it’s changed my life and made me realise I shouldn’t take my health for granted.

    Omg. This video just fixed my lateral pelvic shift! How do I strengthen the areas that allow this shift to happen? How do I prevent this? I’m an active duty Marine and I’ve had this shift since squatting 4 days ago. I can’t even drive to work it’s that bad!

      maintain extension as he mentioned at the end of the video. this usually does well for prevention. If that back bent forward just ever so slightly during your squat, that was likely the culprit, so just make sure you keep a super straight, even slightly extended back during squats. Might even consider an increase in reps and reduction in weight just to reduce compressive forces on the spine - I know it feels good to squat like that, but after a while it might not be worth it for your spine. I've since converted to standing squats with the barbell. Still gets a good workout on the legs, but less of a chance for the back to dip forward as you're upping the weight.

    Way to be so thorough with your video. I'm going to start this exercise today I have this issue and it's been with me for a month now I haven't been able to work or barely walk this whole year sounds funny but it's true no doctor has given me no solution I've been YouTubing my own remedies. So far you seem more along the lines of legit!

    Excellent advice, clear, precise, and no bullshit! I saw loads of professional physios, and read loads of suggestions which did not do much good, with this, within a few hours I have seen progression, thank you. One question however, now I am feeling better, once I carry out the back extensions, are there any suggestions for the best physical training to get my back, and the rest of my body to a standard where I can avoid any recurrence please? I have read loads of conflicting advice and its all rather confusing to me. Many thanks keep up the good work! H

Now, I'm NOT qualified to answer this but I do have the same buggered back as you (ie leaning towards the side WITH the pain) so I'm just going to try this. Try at your own risk I guess...
      The pain in my back is on the same side as the way I'm leaning. The pain, I'm guessing, is caused by squashing something in your back. So do the same leaning exercise so that you move the pain centre towards the wall while keeping your feet out from the wall. That should, in theory, open up the area that hurts and stretch it. I'm trying to eliminate the pain by doing this. I've done one set of 12 reps and it seems better already so I'll keep going. Maybe a physio could tell me if I'm about to cripple myself. If not, I'll carry on.
      Update: Been doing this on and off all day. Seems to be getting better! Basically, feet 12 inches or so from the wall, painful side of your back towards the wall, elbow on the wall and try and touch the wall with the hurting side of your back. Good luck people. Straightness beckons!

      Joe Smith Hi Joe, just discovered this video and seen your comment. My right side of pelvis is raised (left if looking towards me) with pain in both sides but mainly right lower back and sacrum. Should I be leaning against a wall with my right side and pushing from my left?

    I've seen a few videos with this wall exercise and this is the best of them. It's got more info and pointers. Have been crippled up and crooked for a few days but did this yesterday and woke up today with a nearly straight back, much less pain and able to walk way better. Liked and subscribed. Thanks for putting this up. You really know your shift!

    Wow! 10 of these and my back was straight again. I had major back pain, noticed in the mirror I was totally crooked,.I figured i would need surgery or something. This actually straightened me out. Hope it's fixed.

    My shoulders are shifted to the left and the pain is on the lower left part of my back as well with no pain in the right side or the legs. Is it still safe to do this exercise in my case? Thank you for the videos I pray this helps since this is the most miserable I have ever been. Lol

      The only way to know if this will help is to try it. If your symptoms become worse, then stop. Worsening of symptoms would include the pain becoming going down your legs. If it temporarily hurts worse in your back, that is normal, so long as your movement becomes less when you are done with the technique. You may have to try a couple of sets of this.
      If your shoulders are shifted left, then you'll need to do the exercise to correct your shift, so your left side would be against the wall.
      If that doesn't help or makes you worse, then try this exercise. ua-cam.com/video/45FehROHuII/v-deo.html

    Thanks doc- this helped me. I didn't have a disc problem, it was an SI-joint issue, but this exercise helped me to correct my shift and I'm back in the gym (carefully!) just 5 days after my injury.

    I’m going to try this..ive been in this lateral shift for 3 years spent loads on chiropractic care that did nothing ..I’m literally worn out from the pain..i take large doses of Advil n still I’m struggling to even walk it is so bad and I’m not out of shape either but I feel like I’m uncoordinated and my gait is wrong. I will come back with an update after ive done these enough to see some improvement.

    My pain is on my left and my shoulders are shifted to the right but my right hip is sticking out and idk which side to do the exercise on

      Thank you for your question.
      If your shoulders are shifted to your right, then you will place your right shoulder on the wall and push your right hip into the wall.
      You are trying to correct your right sided shift, so you need to move your shoulders to your left. So place your right shoulder against the wall and push your right hip into the wall.
      Try this and let me know how it goes.

    Is there a way to prevent this lateral shift? Every time I have ever "thrown my back out", it has always been this lateral shift and I am usually down for several days. I watched this video, did the stretches, and am actually able to stand up straight again. I still have residual pain, but nothing like yesterday when it happened. Now, back to my original question, is there something that can be done to either prevent, or minimize the changes of this continuing to occur? Thanks.

      Yes. When you are not shifted, I would suggest doing an exercise called "Extension in Lying." Try doing 10 reps of that exercise daily and that might help.

    I have this type of back pain. Woke up with it today. 😔 The back is tilted to the right. The exercise gave me a great relief, thank you. Do you know after what time this type of pain goes away? Does it take one day, a week or longer?

      It varies. Some people are over their pian in 1-2 days. Some it takes longer. It largely depends upon what you do in betweens sets of this exercise. A person who has to sit a lot and with poor posure will likely take longer to get better. ~Sam S.

    I don't have insurance currently and thought I was going to croak over and die from this pain. Tried doing all sorts of lower back exercises in the past which admittedly didn't really help much. But I can actually feel some improvement starting to happen from doing just 3 sets of this exercise.

    I have went through the lateral shift a few times this past year but this one is the worst. I tried this side glide this morning. Yesterday could hardly walk without pain. Today I noticed it is better. After performing this for a couple of reps of 10 times..1) how many times a day? 2)what is the process for icing? Laying down and ice bag or sitting up? or none at all?

    Thanks for posting this. I have several questions. I've been lateral shifted to the right for about two months. I have virtually no back pain but a lot of pain down into the left thigh even to the top of left ankle sometimes. I just started this correction today and have done it several times. Should I only begin the extension in standing when I'm visibly straight or should I do it after every set of pushing against the wall? I have not yet done a set where I look any straighter.
    Also, in the video, it looks like the extensions you're doing are slight tilted back over your right shoulder as opposed to straight back. Am I just seeing that incorrectly?
    Any thoughts at all about how many days of constantly doing this it may take? Thanks for your time.

      Hey Alison - did he ever reply to you? I am in a pretty similar situation only it's my left side. I am noticing a slight improvement in my condition - Though I started doing these the day after I noticed the Lateral shift.

      Yes. You nly progress to extension once you are no longer shifted.

    When you doing the push into the wall with hand on hip my outside knee is tending to bend a little ..is that correct?
