The Pakistani submarine which sunk INS Khukri was sighted in sea by our anti submarine helicopter 2-3 days back. At that time anti-submarine helicopter were not armed with missiles. It was reported to Indian Navy, but it lost a golden chance to sink it. Indian Navy had not pursued that submarine seriously to sink it, as it was in deep planning to attack Karachi n do blockade of it. Pakistan Submarine went in hiding towards Gwadar port that time

Actually innitially India was reluctant to open Naval war. None of Indian Admirals were in favour of Naval conflict with Pakistan.
On 9 december 1971 Pakistani navy Submarine PNS hangor sunk Indian frigate
(Type 14) INS khukari with 117 Crew life casualties . That was provocative act of Pakistan. It worked as fuel in fire. It was the real cause of Indian fury.
There after India left with no choice other than taking revenge using her Naval arm at western region (Arabian sea).
India indeed took a furious revenge by sinking more than 10 Pakistani naval crafts . India fully ruined Karachi harbour.
Pakistan's biggest mistakes
1) Opening western front when they were loosing in East Pakistan.
2) Sinking INS Khukari unnecessarily despite knowing that was doing routine patrolling only.
3) Wrong assessment of India' reaction or retaliation.
PAKISTAN opened naval war. Because they were taught about false or history of fake achievements in Operation Dwarka they did 6 years ago in 1965 war. So this time they were too over confident to estimate Indian naval muscles .

As mentioned in it, Pakistan Navy and PAF should have close coordination in keeping an eye and to obliterate the missile boats but lack of coordination resulted in these disasters. PAF didnt have the night vision capability to track and destroy enemy vessels. Perhaps, the effectiveness and danger of these fast missile boats was not fully understood at Pakistan Navy !
These boats are for defence not offence but indian navy professionally used it as offensive weapon even praised by russian officer
