So if you load a game and when you come back after your 1st Mayhem reset and its only 2 hits. Is it OK to quit game and come back in (like you're farming loot) or do you have to shut down game entirely and come back in?

yes that works too, you can use anything setting in Mayhem to reset the lobby. You can even turn it off and on again
