Better Stream: Optilume for the Treatment of BPH

- In this episode we discuss Optilume, a novel treatment for the urinary symptoms experienced by men due to an enlarged or obstructing prostate (BPH). In previous episodes, we have covered a variety of other BPH treatments ranging from the historical gold standard (TURP) to minimally invasive surgical therapies (MIST) such as Rezum, Urolift, and iTind to more specialized treatments such as HoLEP and robotic simple prostatectomy. So how does Optilume fit into this spectrum of BPH treatments? Does it really stand out from existing treatments or is it just another procedure to add to the list of options in treating BPH? How is the procedure performed? What are the risks and benefits? What is the recovery like? And, of course, how effective is it? To help us answer these questions, we are joined by a true expert.
Dr Dean Elterman is associate professor of urology at the University of Toronro. Dr. Elterman completed his medical degree followed by residency in urologic surgery at the University of Toronto. He became a fellow of the Royal College of Physicians and Surgeons of Canada in 2011. Dr. Elterman completed a two-year fellowship in Voiding Dysfunction, Neuro-Urology, Female Urology and Pelvic Reconstruction at Memorial Sloan-Kettering Cancer Center and New York Presbyterian Hospital/Weill Cornell Medical College in New York City. Dr. Elterman is a member of several professional societies including the Society of Urodynamics, Female Pelvic Medicine and Urogenital Reconstruction, the Society of Genitourinary Reconstructive Surgeons, the International Neuromodulation Society, and the International Society of Men’s Health.

I had this Optilume for BPH ON MAY 15, 2024. POST OP HAS BEEN INCREDIBLE. I've had no pain at all whatsoever. A few bladder spasms sisided by day 3 and I still have some but very little blood in my urine. Because I cannot take NSAIDS to reduce swelling I'm not at full flow but I expect that will change in due course. I researched this for months prior to procedure and everything I hoped for has occurred. I expected some pain but absolutely none, incredible. I haven't used anything for pain only one dose oxybutinin to help with spasms prior to removal of the catheter. I removed the catheter my self with little discomfort. My catheter was a 22fr prelubricated silicone cath with 30ml Balloon. There was discomfort with the catheter but not what could described as pain. I hope anyone thinking of doing something about you enlarged prostate, benign prostatic hyperplasia, looks at this first. The others are inferior in many ways. Day 3 I'm out to mow the lawn, wash my car and take the dog on our daily mile walk. Incredible.
This is encouraging. Trying to get it in Ohio but cannot find anyone performing it yet.
I'd be interested to hear a current update. I guess I'm not understanding how the prostate wouldn't just eventually go right back into constricting the urethra. Widening it out temporarily doesn't seem like that would work at all long term.

May I ask how old you are? I am 80 and wondering if I can be benefitted without complications. I have been told that general anesthesia is not recommended for elderly patients for such treatments.
@@metoymi1910 this a twenty minute procedure that can be done without general anesthesia. I'm 67 and was also concerned til I had an appointment with an anesthesiologist then I felt assured and proceeded with general. I'm at 10 weeks and it's just incredible painless as in not any sensation of the treatment having been performed. I can sleep now four hours at a time and sexual health is improving too. I hope it will help you.

Had this procedure done recently in Shreveport. Self removed catheter at 4 days post surgery.
Advised to stay on Flomax for 6 weeks. However, I tried delaying a dose the other day, and it was a bad experience.
At 1 week out, still start urination with dribbles of blood typically and release of small blood clots sometimes, as expected.
Never took pain meds. Disliked wearing a catheter. The first 24-36 hrs after surgery, it seems that one just pees pure blood.
Once the catheter is out, 4 days after surgery, life is good.
Can't wait to get off Flomax and test flow rate.

@ByDesign333 Almost 4 months post surgery report: no blood after 13 weeks, no Flomax, pee flow good most times, only up once a night typically. The only side effect is slight incontinence, so I wear a #2 ladies pad, as they are less expensive. I'm hoping the incontinence will resolve over time?

How long is this supposed to last? I've heard 1-4 years. When it stops producing positive results, can you have the procedure again? Does anyone know if Medicare covers this procedure?

@hogtide5738 Long-term data as to how long it lasts is not available, and yes to remaining questions.
I'm hoping it lasts 4-5 years. I read the average Prostate grows at a rate of 1.6% a year.
I'm thinking a combination of PAE and Optilume BPH procedures would be better; however, Urologist don't do PAE, and my surgeon was unfamiliar with PAE.

Dean' s office never picks up the phone and if you can get someone that will pick up they never call you back when they say they will. Skip that doctor
