      I totally agree with your list of outstanding physicians and researchers. Hopefully, within the next few years (probably longer), these critical thinkers will DEMOLISH the outdated and bogus "Lipid-Heart" hypothesis. Our main challenge is to get Big Pharma and Big Media and BIG MEDICINE to admit they have been wrong for at least the last 60 years and have been ruining the metabolic health of humans around the world.

    I really like this Doctor I've been following him for a few years now. I was always low fat, higher fiber, higher carbs total cholesterol level always 180 and got sick and feeling like crap. I switched to ketogenic diet and intermittent fasting and within a few months starting feeling better. Now 3 years later on an animal based whole foods diet I'm feeling fantastic and have strength back. Total cholesterol now 280, triglycerides down to 50, HDL 70, excellent ratios and feeling strong and fantastic! Don't fall for the low fat plant based BS ! The so called experts pushing that want you weak and sick. And I never get sick anymore ! My plant based low fat friends are always getting sick.

      TG/HDL is an easy proxy for the quality of LDL (high is oxidized).
      0.71 is a good ratio.
      The toxic Omega 6 seed oils turn the cell membranes into kindling and the toxic non-food iron comes along and oxidizes the kindling and causing a chain reaction of destruction.

    I had an appt with my Md last week to discuss my “high cholesterol”. I told him that I wasn’t concerned. As I explained why, my LDL is high, HDL was good, and my triglycerides were low, HbA1c was 5.6 (ideally I’d like it about 5.4). He agreed with me that he thinks my cholesterol was great. I was surprised.

      I just visited my primary care doctor, and I wished he would have understood like yours, but he didn't. He had a fit! My A1C went down from 5.7 to 5.4, which I know is great, but he said that I traded one issue for another. I told him I appreciated his opinions on my diet, but I wasn't changing and he can put that in my medical record. He was offended that I spoke out against his opinions and advice.
      I reminded him that I listen to what he has to say, but the final decision to what happens to my body is mine alone. I truly believe low carb, keto, and carnivore have changed my health for the better. If I leave it up to him, I would be popping a statin.

    Best in depth discussion ever from a real cardiologist MD doctor I have heard in life. Not one general MD or cardiologist would ever preach this truth. They push dangerous big pharma drugs like statins not health changes via diet.

    And LDL goes dramatically UP while fasting, allowing it to go to work repairing the body and doing all its other amazing work! Some benefits of occasional extended fasting and lowering carbs in the diet: High blood pressure is lowered to normal levels very quickly while fasting. Fibrosis/scarring is reversed over time, including in the heart and lungs.
    Fasting stimulates phagocytosis, the ingestion plaques, growths and pathogens by the immune system. This will also remove spikes quicker, whether natural or unnatural in origin!
    Blood clotting is lessened.
    Blood sugar and insulin are lowered when fasting, reducing inflammation and allowing the immune bodies to move freely through the body.
    Vitamin D plasma levels are increased as fasting improves metabolic health, and vitamin D in turn increases autophagy.
    T cells and T reg cells are vital in fighting cancer, autoimmune disease and infections but as we age the thymus stops making as many of them. Fasting releases stem cells, which then can become new T cells. It also releases growth hormone, which regenerates the thymus itself!
    Fasting restores NAD+ and increases nitric oxide release to open blood vessels.
    Your body recycles up to 1/3 of all immune bodies in a 72h fast, rejuvenating your entire immune system. This helps prevent the onset of new autoimmune conditions, which develop through a leaky gut and damaged immune system.
    Fasts from 36-96 h increase metabolic rate due to norepinephrine release!
    Fasting increases anti-aging Yamanaka factors and increases average telomere length in stem cell pools.
    Fasting can help with MS, Depression, BPD, Autism and seizures.
    Fasting restores your circadian rhythm to normal over time.
    When you move out of MTOR your body shuts down the building blocks of the cell required for viruses to replicate.
    The hunger hormone ghrelin also lowers with extended fasting and rises from dieting.
    What breaks a fast? Anything with protein or carbohydrates in it will break a fast but most teas and herbs are OK. Supplements and meds often break ketosis directly or contain a filler that will. Many meds are dangerous to take while fasting.
    Does fasting lower testosterone? No, it raises it when the fast is broken by increasing lutenizing hormone. Fasting also increases insulin sensitivity, which helps with muscle building.
    Fasting activates autophagy (literally self eating). This will cause cells to recycle damaged proteins and foreign matter such as viruses.
    Lowering insulin via fasting virtually eliminates chronic inflammation in the body.
    Weight loss from daily caloric restriction has 1/4 to 1/3 of the weight lost as lean tissue while many studies show fat loss from 36 h fasts without losing any lean tissue!
    Fasts of 36-96 will not affect short term female fertility or affect menstrual cycle. They also may increase long term fertility issues for some women.
    Fasting reduces pain and anxiety by stimulating the endocannabinoid system, just like the effect of CBD oil
    Just 24h of fasting can cut your leptin levels in half! This reduces leptin resistance, which impairs immune function.
    Stomach acid is reduced over time while fasting and can allow for the healing of treatment resistant ulcers. Some patients may need continued acid reduction medication while fasting.
    Your brain also prefers to burn ketones at a rate of around 2.5 to 1 when they are available in equal quantity to glucose. Except for brief periods of very intense exercise, your body mainly burns fats in the form of free fatty acids.
    It increases mitochondrial function and repairs mitichondrial DNA, leading to improved ATP production and oxygen efficiency. Increased mitochondrial function also has the added benefit of increasing your metabolism, fighting infection and cancer prevention!
    Fasting releases BDNF and NGF in the blood which stimulates new nerve and brain cell growth. This can help a great deal with diseases like MS, peripheral neuropathy and Alzheimers.
    When not in ketosis, the brain can only burn carbohydrate, which produces a great deal of damaging ROS the brain has to deal with.
    Fasting also increases telomere length, negating some of the effects of aging at a cellular level.
    When you fast, this stimulates apoptosis in senescent or genetically damaged cells, destroying them. Senescent cells are responsible for many of the effects of aging and are a root cause of the development of cancer.
    A fasting mimicking diet for 3-5 days in a row also provides many of the same benefits as water fasting. FMD usually has 200-800 calories, under 18 g of protein and extremely low carbs.
    Exogenous ketones can aid with fasting, making it easier in healthy people and allowing some people with specific issues to fast in spite of them without worrying as much about hypoglycemia. They also help with dementia and many other issues even if you take them while not fasting!
    Glycine and trimethylglycine can also be useful supplements while fasting that won't break ketosis and have many benefits.
    Children, pregnant or nursing women should not fast for periods longer than 16 hours. People with pancreatic tumors or certain forms of hypoglycemia generally cannot fast at all. Type 1 diabetics can also fast but it is more complicated and should be approached with caution as it could lead to ketoacidosis. If you experience extreme symptoms of some kind, especially dizziness or tremors, then simply break the fast and seek advice.

      I try and keep things simple myself. I just think about keeping the mitochondria in good shape. (for endurance on the bike, staying lean and of course, health)
      Dr Matthew Phillips from New Zealand is a neurologist who uses Ancestral Strategies to help with Cancer Patients. Well worth catching his talks here of UA-cam as well as his own website.
      He recommends a 48h fast every week, which I have started to do. A 5 day fast as well from time to time (especially timed to work with Kemo therapy if you have cancer). This really does work wonders with mitochondrial health if you are healthy. (With cancer is starves the tumor of glucose and boosts the body's resilience to the Kemo poison, a double win)
      Haven't tried a 5 day fast yet. But keen to have a go. I use a new Keto-Mojo meter to track my GKI when doing these extended fasts just out of curiosity to see what’s really going on, fascinating!

    Thank you so much for interviewing this genius guest & important topic! I've been keto/carnivore for several months & I've never felt better. But my LDL & cholesterol are sky high, as well as ApoB. But HDL is wonderfully high & triglycerides very low. I needed this reassurance that I'm on the right track.       Same here, but I'm wondering why our apob remains high.. according to this guy we already have the begining of a blockage somewhere and the ldl continues to oxidize trying to repair it. Did I understand that right?

    The mind of an engineer is a beautiful thing. HOW DO THINGS WORK? That’s what we need in medicine. Root cause analysis. Not simply changing a number to “fix” an unproven problem. Doctors seem to know what something does but not how it does it. Say “Mevalonate Pathway” to the average doctor and watch them look confused.

      Kathy, all I had to say was "proteolytic enzymes" and I got a red face and a deer in the headlights look for a solid 30 seconds. He responded "yes" when I asked if he was familiar and was stumped when I asked his opinion. At least he didn't say to avoid them.

      @@porkpie2884 I started engaging in muscular endurance training. My performance was significantly better eating carbohydrates compared to when I was on keto, which lasted 3 years for me. Because I do high repetition workouts, i turn over glycogen daily and still have low triglycerides and in range LDL.

    Few years ago, my endo told me he was surprised I hadn't experienced a heart attack. Sent me for a calcium score scan. It came back 41. He couldn't understand how it's possible. I had stopped eating seed/manufactured oils 10 years prior.

    Watching this for the 3rd time. Dr. Ali is so intelligent. I wish I had a doctor like him. I would feel more safe healthwise in making decisions with regards to my health at any given time.

    This has happened to my identical twin brother and me. After my HA in the hospital I was given Lipitor. My reaction to it was to run a fever. Each day I was on Lipitor my fever got higher and ran longer until on the 4th day they stopped the Lipitor. A month after the HA I asked my cardiologist to put me on a statin. He put me on a low dose old statin. After a few months he doubled the dose. I then started to experience AFIB. I increased CoQ10 and I went for years with no AFIB. I stopped the statin due to muscle pain after 4 years. After two years my cardiologist put me on Rapatha. My LDL went from 129 to 59 in a week. Within that week I went into AFIB. I stopped the Rapatha. I went years without AFIB. My identical twin brother was put on Rapatha a year after me. He had never been on Rapatha, did not have heart disease. Very quickly he went into AFIB. I contend it was not my reaction to the statin or Rapatha that put me in AFIB, but the low LDL.

    For cycling I can say that carbs, at least for me, interfere with glucose balance. In zone 2 I can do 4 hours with a flat blood glucose level and that is in the end determines how you feel (check out Tim Noakes on this). Even more the rate of change of the blood glucose drop. When I went carnivore my recovery really surprised me. Nowadays under 2 hours I never eat, not before and during training. I see no difference in heart rate and power output.

      I qualified for the Boston Marathon on carnivore diet. I have 365 LDL and my recovery is fantastic. My doctor wanted me on statin. Needless to say I don’t use a doctor anymore

    new generation of doctors have the same view BUT the older generation doctors asked me to take drugs/statin to lower my LDL ......... my triglicerides level are healthy, healthy glucose level. isn't it alright? say no to anti cholesterol drugs..

      Statins increase the calcium scores in arteries and increase diabetes risk - both high risk of heart failure.

      Refuse all cholesterol reducing drugs. PCSK9 inhibitors are the latest drugs that will lower your ldl way down to 40 or less. Don’t be a test subject/lab rat. This crap hasn’t been tested on anyone except the people currently taking it. Good luck fighting the bully doctors.

    I adore Dr Ali and I am so appreciative of his information and sharing what he learns and believes.
    One thing I wish is that someone would address if a person already has confirmed coronary artery disease and have stents, is it still wise to mainly focus on keeping blood sugar down and in control, by eating very low carb, good fats, no seed oils, no processed food and not be concerned with anything other than triglyceride/HDL ratio?
    Thank you for interviewing the people who are working to change the traditional "wisdom".

    This is one of the best podcast with a Cardiologist and researcher I have seen on metabolism with focus on lipid levels! I am one of the individuals with familial hypercholesterinemia gene, never had high LDL Until I became post menopausal even though I am on low carb diet with Keto cycles for a decade now. I have low Triglyceride levels ( 70 and lower, high HDL, borderline high LPa and ApoB though). I will be watching it. I am glad you pointed out the brain works best on Cholesterol not only on glucose (We have insulin receptors in the brain too.)

    Listening to you is very good. I have 80 four-year-old relative.That doesn't like the way her health is going.But she also does not want to change her lifestyle or eating habits. It gets frustrating but I guess I'm at the point right now where I have to tell her the bald truth. Thank you for all the information.

    56:25, Does fat in food contribute to high fat in the blood stream? My triglycerides levels went down almost by 100 points from eating low carb and high fat.

    Thanks so much for replying to me. I am so happy to from you doctor. I refused to take the statin because my doctor prescribed for me when he saw the test results was 165 the bad cholesterol and the rest I was fine. I am 59 years old and I am active and healthy.

    I am only 22 minutes into this video, and feel the need to continue listening. Recently, my cardiologist office said I need to get my LDL (126) to 35. HDL 63, TC 214, while on Praluent , all lower on Repatha. Nevertheless, I was alarmed, and thought to myself that it was not a good idea. Thanks for validating my instincts. I do have comorbidities, but I am active, lots of vegetables and occasionally meat, scratch cook, 152 lb, 78yo, 5.65 height.❤

He ignored my complaints of numerous PVCs so when I researched and started Magnesium on my own he became annoyed and said, "Internet" and offered me a beta blocker, which he would have had to restrain me to take. Anyway, I told him no, that the Magnesium was working just fine, to which he said, "Yeah that will work.". It's sad really, but pretty typical of the cardiologists I have seen. I decided I didn't need to drive an hour for bad care as I could get poor care closer to my home. At least his office put effort into seeing that I got the Repatha for my cholesterol, as my body just could not tolerate even the weakest statin anymore. Switching to Praluent was ineffective even with walking 2-6 miles a day, so the office is working on getting the Repatha reinstated. I realized years ago I have to take care of myself. Strangely, no doctor has offered nutritional advice, but I garden, which is helpful.

      LDL that low will send you to an early grave.
      The papers coming out now say the lower LDL, the higher mortality.
      The higher LDL is more protective.

      Flip that around. Eat lots of beef and not a lot of vegetables if you want to be strong and healthy again. Veggies have lots of defense chemicals (ie poison). Some more than others. Oxylates and lectins are just the ones we know about. Just try removing them and see. Assuming you’re already low carb. If not do that first.

    I’d love more explanation on cholesterol absorption vs synthesis. It has been proffered by many that little dietary cholesterol is absorbed and most (80%) is synthesized in the body.

    the problem is that doctors do not use the complete LDL screening exam to identify the small particle LDL versus the harmless larger LDL particles.
