Pre-conditions for filing online PF claims:
1) UAN should have been activated.
2) Member’s AADHAAR details should be updated and verified in UAN Portal.
3) Bank account along with IFSC code should be seeded in UAN Portal.
Follow the steps below to submit your PF claim online:
1) Login to Member Interface of Unified Portal (unifiedportal-mem.epfindia.go....
2) Go to Online Services Claim(Form-31,19,10C & 10D).
3) Enter last 4 digits of your Bank Account and verify.
4) Click on “Proceed For Online Claim”.
5) Select PF Advance (Form 31) from the drop down.
6) Select purpose from the drop down.
7) Enter amount required and Upload scanned copy of cheque and enter your address.
8) Click on “Get Aadhaar OTP”.
9) Enter the OTP received on Aadhaar linked mobile.
20) Claim is submitted.

Actually you hided the address column which is the main part of claim. Where you need to enter the Locality address which is restricting to complete the form. If your providing information to public provide proper steps and instructions. Waste of watching this video. You cannot submit the claim.
