When someone becomes a Hospice of the Northwest patient, we assemble a dedicated care-team to support their comfort and dignity.
Patient care teams consist of a board-certified physician, registered nurse, social worker, and spiritual counselor.
This interdisciplinary team works together to make sure every patient has a personalized care plan specific to their individual needs and goals. Patient care teams regularly communicate with each other to ensure their approach is consistent.
Sometimes additional service providers are recommended. This may include integrative therapists, hospice care aides, and trained volunteers.
