Honestly most of these are due to poor maintenance and worn-out engines -20°c should start on the key even -30°c is only a slight struggle. That includes old diesels.
It depends on many things. Most engines will start super fast because they have fresh oil, fresh winter diesel fuel and are still warm from the previous drive (the engine will still retain some heat for a day or two) and a charged battery. Try it with a car that has been sitting for a month or two and it won't be that easy.

People should stop revving the engine after starting it cold. The pistons have not have time to warm up and expand yet so revving the engine can cause knocking and really mess up the piston and cylinder walls
@Larry Butler I’ve got a 1990 MB 190D (w201)! If one or more glow plugs are dead, the glow plug indicator on the dashboard will not light up! It’s the same like in all other MBs I had before since (w123 and w115)! Maybe it’s different in your country but I suspect the w116 is more of an engine swap where the preheating control or the preheating relay was incorrectly connected. Is that possible?!
Buy a preheater, push a button on a remotecontroll, 10min before you want to drive an you have a warm car and a warm motor. You would think these guys would allready have one, living in these cpnditions.

I'll replace the starter rather than the engine. Starting fluid is horrible for engines. Besides the starters on a diesel are going to last longer by holding the key than stopping and trying again. The pressure from firing will work the starter harder and more likely to kick back and break the starter clean out of the engine. Your supposed to hold it better for the starter and more likely to start as ur building more and more heat till it gets to the point that it starts
