There's probably a handful of ways to approach that in Nomad. Using an ellipse shape to boolean subtract the cavity would be the way I would probably approach it. If you watch my Chain Chomp Sculpt video, I show a very exaggerated version of that. Another method would be to use the Inflate, Brush, or Clay tools with Sub turned on, this will easily create a cavity. Sub is under your pressure sliders in the left tool strip. Hope that helps.
