For slab design purposes, they are always inner face to inner face of beam. In plans (blue print) however, dimensions are given normally center to center for inner beams or elements. For perimeter beams, mostly, dimensions are from center of inner beam to outer face of perimeter beam.

Is it correct to start the rebar installation at the middle of the span then go to its left and right sides until reaching the edge providing 20 mm concrete cover at the vertical face of the slab? Are shorter bars, always at the bottom and longer ones on top of them for slabs and footings?
Shorter bar at the bottom for positive moment. While it is at tho top for negative moment. Temp Bars may be positioned at mid- Thickness of slab. But easier if in contact with the main bars. Temp bars above shortt bars for positive moment while below short bars for negative moment.
It depends on the preparation of the base and on whether it is a floating slab(no flexure) or slab connected to grade beams ( may be subject to flexure)
