Thank you so much for all of that information. You brought up things that I hadn't even considered and should have as I am a qualified Nurse here in the UK. I have wanted upper eyelid surgery for some years but could never afford it but now I'm in a situation that my Optometrist has asked my GP to refer me for Ptosis surgery which hopefully will be done on the NHS. An oculoplastic surgeon is especially important for treating drooping eyelids, as general cosmetic and plastic surgeons are not trained to address the levator muscle that lifts/opens the upper eyelid. Often in these cases, only skin removal is performed, which can lead to complications associated with a skin shortage.

You are very clear and informative. I have very hooded eyes which are now causing my top lashes to on the outer corner to be pressed into my bottom lashes. But I'll be honest after hearing of the mistakes that can ocur, combined with the many people who have commented relating bad outcomes I'm afraid to have surgury. It's a sad lose/lose situation. Sigh
Hi @lindawitt4661, Thank you for watching. I do understand your hesitation. As explained in the video, risks are reduced by the expertise, and experience of the performing surgeon. Complications are most often due to surgeons performing surgeries outside of their expertise, and/or are not yet experienced in a particular surgery. The difference between successful cosmetic eyelid surgery, and complications are mere millimeters, so planning, expertise, and experience are all needed for a successful, and safe outcome. I had upper/lower bleph just over a month ago. I thought the tightness would go away by now, but it's still very tight & I can feel the tug/pull of the incision each time I blink. I hope it will go away. My eyelids are also still numb. And, I can just barely close my eyes but there's no extra give so I can't scrunch my eyes closed (like I used to in the shower to keep water out). I guess that's the "new" normal. Just wish this tight feeling around the incision would go away. I heard it goes away in 2 weeks but like I said it's been just over a month.

Hi Julia Lee, While I do know some doctors from around the country, we all have different practice focuses and specialties. I suggest you look at the before and after photos of prospective doctors, ask them if they can differentiate eyelid ptosis (eyelid muscle issue) from dermatochalasis (extra eyelid skin), and choose a doctor with whom you are most comfortable. I am a 32 year old woman who had a horribly botched blepharoplasty. My upper lids were cut way too short and my lower lids were retracted. I had successful lower lid retraction surgery to bring my lower lids to the right position. Now, my eyes are both able to close, but they still both come open at night from severe skin shortage on the upper lids. I strongly feel I need skin grafts on the upper lids to give myself enough tissue. When I look downward, skin only covers half the eyeballs, unlike most people whose eyes are all covered with skin when they look downward. It seems obvious to me that I have a significant skin shortage but some local doctors look at me and after measuring the distance between my eyebrow to my eye say I have enough skin even though my eyes still open considerably at night and continue to let me suffer. Why do they say this? Is it possible to get a good cosmetic result from upper eyelid skin grafts when all the healing is done? I heard about lasers and PRP being done to help with the healing.

Dr. I have blepharoplasty 2months back now I have hypertrophic scar in the lateral border of the incision side where they tie sutures and there is mild pain in that area also. I was diagnosed hypothyroidism after the surgery and I'm under medication for that. is this hypertrophic scar related to my hypothyroidism? And will this hypertrophic scar reduce by itself.

Thank you for this information! I was wondering, can blepharoplasty be done on just one eye? I have some pretty asymetrical eyelids, and only the right one is uncomfortable, so it would be fine with me just to leave the left one as is.
Hi C L, It depends on what is determined in a physical examination. Eyelid height asymmetry of 1-2mm between the two eyelids is within a normal range that is unnoticeable by others, so I wouldn't recommend surgery for slight asymmetry. If it more than that, an examination is needed to determine if it's caused by something other than bone structure. I hope this clarifies things for you.

Dear Dr Prasad, I underwent a transconjunctival blepharoplasty three and half weeks ago but I am very depressed and unsatisfied with the outcome. I am now left with hollow tear troughs even though the eyebags are gone. They make me look tired and older. Hi Chin Kaar Ti, Sorry to hear about your problem. There is an art in lower eyelid surgery in leaving a little fat behind to prevent a hollow appearance. Injectable fillers may help, but I would need to see in an examination where the hollowness is before I confirm where I would place them. There is an art to filler placement as well for best results. Hi @chhayabaghel9475 , The frontalis sling is a last resort procedure only used when there is little to no levator muscle function to fully lift the upper eyelid. The frontalis sling bypasses the levator muscle, and uses the frontalis muscle of the forehead to lift the eyelid. Not only is there a learning curve to use a different muscle to blink, but this increased activity of the forehead can cause premature creases in the forehead. I hope this clarifies things for you.

She still have headaches after 5 months. Eyes a little bit hurting. Dr Prasad I found your video very intersting,I just had eye lid surgery and one eye looks not right either too much skin taken off or the shape of the cut is not the same as the other or both? You need to be patient and follow up with your surgeon as healing continues. Hi ALEXANDER MONDAY, Sutures for upper eyelid surgery are usually removed one week after surgery. The same goes for lower eyelid surgery, but often the incision on the inside of the lower eyelid can be cauterized with a laser, in which case no sutures are used. Upper blepharoplasty only deals with the eyelid skin, and the fat behind the eyelids, not the visual function of the eye itself. Dr. Prasad, i have MGJWS with strong winking and ptosis about 3 mm in resting position. Looking at the mirror, i suspect that ihave above the ptotic eye also exessive fat(dermatochalasis?). The question is, if it is possible to underwent only the blepharoplastix sx, remove thus the fat and not the other surgery, the muscle resection wich is more comlicate. Will this open the lid for 2-3 mm? if requested, i can sent pictures. thank you.
Hi kadi diwup, If you have eyelid ptosis, fat removal alone, and even eyelid skin removal, will not be enough to lift your eyelid by 2-3mm. People with ptosis who only have upper blepharoplasty with the skin and fat removed only will still have drooping. If too much eyelid skin is removed, you will likely not be able to close your eyes fully. If too much fat is removed, your eyelid and brow will look hollow. Operating on the levator muscle or Mueller's muscle are the only ways to properly raise resting eyelid height. I hope this clarifies things for you.

Hi@@kadidiwup1752, Eyelid ptosis is caused by stretched, detached, or undeveloped eyelid muscle called the levator muscle, so fat removal alone won't be enough to change eyelid height. I have performed surgery on patients with Marcus Gunn jaw winking syndrome, but a full evaluation is needed to determine if surgery is the best option for treatment. Hi Rosethmae Guerrero, Sorry to hear about your problem. A frontalis sling may take time to get accustomed to as it's a different muscle raising your eyelid, especially when it's done as an adult. I suggest you take some time to get used to it, and follow up with your doctor if there are issues like the eyelid looking too low, or raised too high. It is a last resort eyelid ptosis procedure, so when it's done, it means other ptosis correction procedures would not have worked in your case. I hope this clarifies things for you.

Hi Build Your Freedom, That would be part of the risk, but such cases are exceedingly rare. Doctors who perform cosmetic eyelid surgery are confident in their dexterity in the eye area as it's highly delicate, and many doctors are upfront in saying that they don't even perform injection treatments in the eye area. There are risks to any surgery, but risks are minimized by the experience of the doctor. I hope this helps you.

The most I am scared because one Dr. told me about the risk of going blind and I was socked. What do you advice? How can I minimize that? Hi Aman Toor, It isn't likely to tighten skin enough to reduce hooding, and Ultherapy is very hot, with limited control in adjusting temperature for thin eyelids, so it may not be a safe treatment. Any doctor who can perform thermal treatments safely in the eyelid area would be able to perform cosmetic eyelid surgery. I hope this helps you.

Hi Kanika Sharma, Upper blepharoplasty is the removal of redundant skin and fat, so it's not the right surgery for ptosis since it is caused by a stretched, detached, weakened, or defective muscle that lifts the eyelid. General plastic and cosmetic surgeons who perform upper blepharoplasty are not trained to treat eyelid ptosis. Eyelid ptosis can recur as the levator muscle that lifts the eyelid is engaged thousands of times a day to blink, so this amount of activity over years can loosen, stretch, or detach the levator muscle that was previously treated with surgery. A proper ptosis examination is needed to diagnose for muscle function, and what type of treatment would be appropriate for you. Thank you so much for all of that information. You brought up things that I hadn't even considered and should have as I am a qualified Nurse here in the UK. I have wanted upper eyelid surgery for some years but could never afford it but now I'm in a situation that my Optometrist has asked my GP to refer me for Ptosis surgery which hopefully will be done on the NHS. You are very clear and informative. I have very hooded eyes which are now causing my top lashes to on the outer corner to be pressed into my bottom lashes. Complications are most often due to surgeons performing surgeries outside of their expertise, and/or are not yet experienced in a particular surgery. The difference between successful cosmetic eyelid surgery, and complications are mere millimeters, so planning, expertise, and experience are all needed for a successful, and safe outcome. I had upper/lower bleph just over a month ago. I thought the tightness would go away by now, but it's still very tight & I can feel the tug/pull of the incision each time I blink. I hope it will go away. My eyelids are also still numb. And, I can just barely close my eyes but there's no extra give so I can't scrunch my eyes closed (like I used to in the shower to keep water out). I guess that's the "new" normal. Just wish this tight feeling around the incision would go away. I heard it goes away in 2 weeks but like I said it's been just over a month.

While I do know some doctors from around the country, we all have different practice focuses and specialties. I suggest you look at the before and after photos of prospective doctors, ask them if they can differentiate eyelid ptosis (eyelid muscle issue) from dermatochalasis (extra eyelid skin), and choose a doctor with whom you are most comfortable. I am a 32 year old woman who had a horribly botched blepharoplasty. My upper lids were cut way too short and my lower lids were retracted. I had successful lower lid retraction surgery to bring my lower lids to the right position. Now, my eyes are both able to close, but they still both come open at night from severe skin shortage on the upper lids. I strongly feel I need skin grafts on the upper lids to give myself enough tissue. When I look downward, skin only covers half the eyeballs, unlike most people whose eyes are all covered with skin when they look downward. It seems obvious to me that I have a significant skin shortage but some local doctors look at me and after measuring the distance between my eyebrow to my eye say I have enough skin even though my eyes still open considerably at night and continue to let me suffer. Why do they say this? Is it possible to get a good cosmetic result from upper eyelid skin grafts when all the healing is done? I heard about lasers and PRP being done to help with the healing.

Dr. I have blepharoplasty 2months back now I have hypertrophic scar in the lateral border of the incision side where they tie sutures and there is mild pain in that area also. It depends on what is determined in a physical examination. Eyelid height asymmetry of 1-2mm between the two eyelids is within a normal range that is unnoticeable by others, so I wouldn't recommend surgery for slight asymmetry. If it more than that, an examination is needed to determine if it's caused by something other than bone structure. I hope this clarifies things for you.

Dear Dr Prasad, I underwent a transconjunctival blepharoplasty three and half weeks ago but I am very depressed and unsatisfied with the outcome. I am now left with hollow tear troughs even though the eyebags are gone. They make me look tired and older. Sorry to hear about your problem. There is an art in lower eyelid surgery in leaving a little fat behind to prevent a hollow appearance. Injectable fillers may help, but I would need to see in an examination where the hollowness is before I confirm where I would place them. There is an art to filler placement as well for best results. What are the disadvantage of frontalis sling surgery

The frontalis sling is a last resort procedure only used when there is little to no levator muscle function to fully lift the upper eyelid. The frontalis sling bypasses the levator muscle, and uses the frontalis muscle of the forehead to lift the eyelid. Not only is there a learning curve to use a different muscle to blink, but this increased activity of the forehead can cause premature creases in the forehead. I hope this clarifies things for you.
Dear sir, a neighbor in our building had eyelid surgery 5 months ago. She did not feel good after the surgery. Had dry eyes, eyes were hurting a little and she has headaches. Sorry to hear about your problem. At 6 weeks post-op, you cannot determine yet if it's the final result due to temporary swelling and ongoing healing. You need to be patient and follow up with your surgeon as healing continues. Dr. Prasad, i have MGJWS with strong winking and ptosis about 3 mm in resting position.
Looking at the mirror, i suspect that ihave above the ptotic eye also exessive fat(dermatochalasis?). The question is, if it is possible to underwent only the blepharoplastix sx, remove thus the fat and not the other surgery, the muscle resection wich is more comlicate. Will this open the lid for 2-3 mm?

If you have eyelid ptosis, fat removal alone, and even eyelid skin removal, will not be enough to lift your eyelid by 2-3mm. People with ptosis who only have upper blepharoplasty with the skin and fat removed only will still have drooping. If too much eyelid skin is removed, you will likely not be able to close your eyes fully. If too much fat is removed, your eyelid and brow will look hollow. Operating on the levator muscle or Mueller's muscle are the only ways to properly raise resting eyelid height. I hope this clarifies things for you.

Eyelid ptosis is caused by stretched, detached, or undeveloped eyelid muscle called the levator muscle, so fat removal alone won't be enough to change eyelid height. I have performed surgery on patients with Marcus Gunn jaw winking syndrome, but a full evaluation is needed to determine if surgery is the best option for treatment. Sorry to hear about your problem. A frontalis sling may take time to get accustomed to as it's a different muscle raising your eyelid, especially when it's done as an adult. I suggest you take some time to get used to it, and follow up with your doctor if there are issues like the eyelid looking too low, or raised too high. It is a last resort eyelid ptosis procedure, so when it's done, it means other ptosis correction procedures would not have worked in your case. I hope this clarifies things for you.

That would be part of the risk, but such cases are exceedingly rare. Doctors who perform cosmetic eyelid surgery are confident in their dexterity in the eye area as it's highly delicate, and many doctors are upfront in saying that they don't even perform injection treatments in the eye area. Does Ultherapy works on hooded eyes

It isn't likely to tighten skin enough to reduce hooding, and Ultherapy is very hot, with limited control in adjusting temperature for thin eyelids, so it may not be a safe treatment. Upper blepharoplasty is the removal of redundant skin and fat, so it's not the right surgery for ptosis since it is caused by a stretched, detached, weakened, or defective muscle that lifts the eyelid. General plastic and cosmetic surgeons who perform upper blepharoplasty are not trained to treat eyelid ptosis. Eyelid ptosis can recur as the levator muscle that lifts the eyelid is engaged thousands of times a day to blink, so this amount of activity over years can loosen, stretch, or detach the levator muscle that was previously treated with surgery. A proper ptosis examination is needed to diagnose for muscle function, and what type of treatment would be appropriate for you. Blepharoplasty is an elective cosmetic procedure, so it's usually not covered by insurance. You may be referring to eyelid ptosis surgery, which can be covered by insurance since vision is affected, but insurance approved surgery usually covers functionality of the upper eyelid over appearance. Sorry, but our offices don't accept any insurance as insurance companies are quite technical and problematic when dealing with a cosmetic medical practice. I hope this clarifies things for you.
