This isn't really as confusing as people think. Double Rows became more common after Yavin and were continuously for years after even surviving until the collapse of the Fel Empire. Single Rows were still a plaque worn after Yavin but tended to be less prominent.
Agreed 👍 having studied this subject, apart from a number of confusing rank insignias. The system is pretty clear to me if you have a good knowledge of Star Wars.

This is, of course, new canon. Old canon never formulated rank insignia correctly. I was able to figure out the original system, but you won't find any public information on it.

He was incorrect about the nomenclature, but close. I would say that the original film is the only that has a sensible system of discernible ranks. You can clearly identify the officers with orange squares as army, blue as navy. Army wear brown uniforms. Navy wear black. Colonels wear a light gray suit and Captains wear the ISB uniform. The senior ranks all appear to wear this or a brown uniform as well. The least amount of squares an officer has is two. From there, you should be able to discern up to Colonel/Captain at least.

They were over done, but they seem to resolve some of the lack of thoughtful design that went into the original designer's work. Ironic, since I believe he was an expert on military uniforms. I wouldn't say they're accurate though since some of Tarkin's aides are identified as generals when they should be at most captains or majors in the old system.
There has been a rumor for some years that the ranks plaques were incorrectly used. Moff Jerjerrod should have been 5 red over 3 blue and two yellow but instead wore a Captain's plaque. The officers in the row behind mainly had incorrect plaques, too. However, at least they were double row plaques, which was the only consistent element.
It's on Reddit as part of Maw Installation's blog. The chart was actually crested by King Doran. I am currently working on a video with King Doran to create a detailed review of his chart.

Casido would have made a fine Captain and then Admiral taking charge as he did while the commadore stayed silent and fleet admiral gorin decided to wait for reinforcements he could have become a Fleet Captain commodore and Admiral if he had the chance to rise to that rank

A generalissimo rank should be created 3 blue and 3 red over 6 yellow plaques - on par as emperor and above grand moff
