This song is essentially an adaptation of the book “Johnny Got His Gun” by Dalton Trumbo. Others here have already explained the plot, so I won’t go into that. Those scenes in the music video are actually direct clips and quotes from the movie adaptation, written and directed by the original author. Fun fact though, Metallica got sick of having to pay royalty fees, so they just straight-up bought the rights to the movie so they could use it whenever they wanted.

So this is a anti war song. The video has a lot of the film 'Johnny got his gun' running throughout. Metallica bought the rights to the film so they could continue to use it. It's about a WW1 soldier who was hit by an artillery shell and was left blind, deaf, dumb and without limbs. It's his struggle for release from pain that his doctors don't realise he's in. the line ' He's a product of your profession, not mine' really hits once you know the context.
I wish the first time i heard rhis wqs with heqdphonea. That S.O.S. at the end would be "creepy" (like you said as i was typing this).... i saw the video YEARS after I heard the song (w/out movie audio clipped in). It was a trip then trying to aort it out... but headphones would have benn way more ominous.
