What makes this bad is Homelander can tell when someones lying. He knew Cameron was telling the truth, but wanted to use him anyway as a hazing ritual.
Homelander can only tell when someone is lying through bodily responses such as an elevated heartrate or differences in breathing. Cam probably exhibited “proof” of his lying because he was scared shitless.

Probably, when he saw proof and evidence, he didn't think it was necessary to really focus in and use his powers. That's the way his powers are presented in the show, small details like Edgars' heartbeat he has to focus in on, but a explosion in the general vicinity of him he can hear immediately. Its the same reason why he couldn't immediately kill Hughie his hearing was impaired from the singing and all the flashing lights overwhelmed his senses impairing his ability to focus. Doesn’t matter if you have x ray vision or be a human lie detector when your super senses can easilly overwhelm you.
The shot at everyone slowly approaching Cole is haunting. Like you are surrounded by a bunch of super beings who are ready to beat the shit out of you. And in the top of that you get mocked by Fuckin Deep.

I think Homelander 100% does know A Train is the leak but just doesn’t care enough yet to proceed with going after him, I think he and Sage have both noticed A train’s lack of interest in killing when Homelander orders him too and are going to continue to test him in these kind of scenarios until A train snaps and retaliates against them.
My best guess is that homelander is finally practicing what he preaches about "supes shouldn't kill other supes" so that they can all be aligned to take over the U.S, or maybe he's doing it cause he wants a better world for ryan/supes. idk tho, still feel like he would've had no issue gutting A-train since he should definitely know about him taking the V. Maybe he's just too unhinged to care
Sage knows and probably thinks it’s more useful to have an “in” with The Boys than turning A Train over at the moment. She could secretly bug him or she could plant false intel that A-Train brings to MM that puts them on a wild goose chase or killed while Vought does something else.

Honestly, I thought for a moment that A-Train in a moment of kindness was going to grab Coleman and run away from the tower. I think Homelander and Sage know about his betrayal but they want to test his mental state to the maximum.
With the way this show literally never holds back, it was unnerving to not see what they did to Cameron or his body after. Maybe they show it in the next episode but it was honestly more haunting to hear a character you’ve seen all 4 seasons get killed off screen like that.
