  • Communication errors contribute to between 70 to 90% of all accidents and incidents on the railway.
    This is why a cross-industry working group was set up to look at how communications could be improved.
    The Communications Working Group (CWG) has already done a lot or work to establish what type of communication problems exist. They are now focussing on new initiatives which improve standards so that formal, structured communications between drivers and signallers, and signallers and track staff becomes the norm.
    This video is one such Initiative. It has been designed for use as part of:
    safety briefings formal training programmes
    the cross-Industry safety critical communications forum The key message is that safety critical communications is not simply about using
    the phonetic alphabet - it is about reaching a clear understanding with the other party, not making assumptions and ensuring messages are repeated back.
    The video includes 4 incidents where poor safety critical communications had potentially disastrous, and in one incident, tragic consequences.
    Scenario 1 - Victoria
    Two passenger trains nearly collided when a driver, who didn't follow the correct procedure, thought he had authority to pass a signal at danger.
    Scenario 2 - Toton
    This incident shows how poor communications between a PICOP and a MMUN handsignaller led to a new worksite not being communicated, causing a near miss that could have had tragic consequences.
    Scenario 3 - Chevington North
    Shows how not reaching a clear understanding with the signaller and repeating back messages, allowed a train to enter a worksite without warning.
    Scenario 4 - Cwmbargoed
    This tragic incident shows how by not maintaining clear and continuous communications, fatal consequences can occur

    I feel bad for the signaller in the 3rd clip. He's just working that night shift. It's not his fault the Picop had a bad briefing!
