They should have done this study including diabetics who never did keto or exercise, they would find interesting results.
I can tell you something for having extensively tested things on my self like I'm a lab rat :
Low carb diet + protein + fats as fuel + exercise LOWERS CONSIDERABLY the insulin resistance and cuts by unbelievable amounts the exo insulin needed, maybe /10 to /15 times.
Problem is doctors even specialist are not working on the terrain and do not (cannot) really know how a body will react in certain circumstances. So they blindly apply what they have been taught like it's just a checkbox test. Diabetic (type 2)? = Insulin in worse case. Period. They should really be open minded and stick to facts when professional and personal experiments are conducted.

“Afflicted by studies with small sample sizes, tiny effects, invalid exploratory analyses, and flagrant conflicts of interest, together with an obsession for pursuing fashionable trends of dubious importance, science has taken a turn towards darkness,” writes Horton.

Isn't this obvious? What a waste of time and money this study was. Anytime someone ingests a drug that they don't regularly has a lower tolerance to it. For example, someone who drinks alcohol regularly can drink a whole bottle of wine before they get drunk, whilst someone who seldom drinks will feel drunk after one glass of wine.
    Same principle.
The theory that you should go off keto regardless of what your blood work is, just to stay carb adapted, is akin to saying you should go and drink some alcohol periodically or shoot up some heroin every now and then just so your body remembers how to process it. Ridiculous

Im my opinion you are correct in the case of alcohol but wrong to compare alcohol and heroin to Carbohydrates.. Carbohydrates are not drugs they are the bodys prefered energy source and are important for the body.. You sound like one of those vegans overexagurating about how eating meat is like drugs or eating a dog or human lol
The human body can adapt to most things, our gut bacteria changes when we eat certain foods.
The Keto diet has its place for fat loss and muscle maintenance but is best used as a short term diet then once body fat is lost. You carb cycle instead of completely removing carbs from the diet and when you eat carbs eat healthy carbs such as sweet potato, quinoa, Oats, Sourdough bread, black/brown rice, whole weat pasta or gluten free pasta even legumes that have additional protein such as lentils, beans, splitpeas and chickpeas.
not white bread, processed cerial, heaps of potatos, rice, pastries, normal pasta, lollies and processed foods
The healthiest long term is a ballenced majority whole foods plant based diet with occasional white meats throughout the week like; wild caught fish and birds like Chicken, Turkey, duck and so on.. Unless you want to be like our closest monkey relitive and eat Fruit, veg, nuts, seeds and insects and other monkeys poop for B12 lol
also no need to continue that silly negitive belief that "women are always right" and men are wrong crap.. People say that in marriage just to avoid arguments lol
what matters in life is to treat others with respect, and offer different veiw points as we can always learn something new.

While under Keto, it seems the insulin (production or function) is no more trained to regulate/respond well and timely when some carbs are being ingested.
My Keto diet includes less than 80 gr. carbs per day.
Following a 4 months Keto diet, I lost weight (78 kg. to 68 kg. - my height is 173 cm) and my HDL went up (45 mg/dl to 72 mg/dl).
That's great and these are the pros.
On the other hand my HB A1C (%) went up (5.3 to 5.7) and my fasting Glucose went up (from 70 mg/dl to 77 mg/dl). I also lost some muscle mass.

Solid video. I appreciate the perspective you bring to what some might use to attack keto/low carb eating-no doubt they will as it seems like a war. I am unclear about the theory of "glucose intolerance" from a low carb/keto diet....we always have glucose in our bodies for fuel and retain the ability to metabolize it. What is going on with low carb people who do an OGTT is that they do not store much insulin as it isn't regularly needed.
When they are given 75 grams of sugar there is not insulin at the ready to bring blood sugar levels down. This is called the Phase 1 insulin response. As the body senses blood sugar quickly rising, insulin production goes into overdrive and this is Phase 2 response.
So it isn't that the body doesn't know how to process glucose properly or that the cells are "intolerant" of it, it is just that there isn't insulin available to move glucoseout of the blood. In essence no frontline troops at the ready. This is why if someone doing low carb is getting an OGTT, they must eat more carbs in the days leading up to the test and the meal the night before or they will get wacky high readings.

It stands to reason that endurance athletes, or any athlete, would not do well on a low carb way of eating. Common sense tells you that. The nutritional needs of athletes are very different from someone riding a desk, driving a bus, long haul truck drivers, school teachers, etc. etc. If the study had been done on various age groups of normal people the findings would have been vastly different. I have been eating Keto/LCHF, even carnivore at times, for 4 or 5 years. Inflammation is almost non existent, weight under control (still have a few pounds to lose), muscle development is on par for the resistance training I do, HIIT and cardio are much better than most people my age (60). No medications, no illness, no need to even visit a doctor TBH. IF and Keto/carnivore are the bane of big pharma and are therefore demonized. They don't want a world filled with healthy people because it reduces their profits. Listen to your body, it will tell you what it needs.

The title is not an accurate reflection of the findings of the study. As Dr Benjamin Bikman, who probably is the leading expert on insulin resistance out there has mentioned in his videos, the cells remain exquisitely sensitive to insulin when carbs are re-introduced to a person following a very low carb diet. This was found where an insulin clamp was applied to those subjects where the insulin removed the glucose from the blood without any loss in effectiveness. The issue is that the reaction to produce the necessary amount of insulin is slower, so the production rate is affected but not the effectiveness of the insulin produced is not affected at all.

What if I already had intolerance when I began Keto 2 1/2 years ago? My body can’t handle anything with more than about 15 carbs due to surgery. I tried cycling off and I was sick. Felt horrible, gained 12 lbs in a very short time, yet I really hadn’t eaten much differently other than adding carbs, good carbs to my diet for 3 weeks. I won’t cycle off again. Keto is my way of life now. I feel great! I’ve lost 63 lbs, (well as soon as I can get this last 8 lbs back off) and at 65 I can workout at the gym several hours a day. If you can't handle carbs, don't worry about eating them. Your body can make what it needs for your brain and your muscles are perfectly happy with protein and fats. As to losing that last 8 lbs., been there, done that thing. Try dropping your fats slightly and upping your protein levels for a month. You will have to play to find the right level, but the less fat means your body will go looking to use the fat on your body. And as to high cholesterol, does your doctor fuss at you like mine does because my LDL is up? My triglycerides are half of what they were and my a1c has dropped to 4.9 and my HDL has raised significantly. Your LDL is just a boat delivering fats to your body and taking them to your liver. A high cholesterol is only a worry if you already have heart failure. Otherwise, it is just a number. Keep doing what you are doing and you will be fine.

You can store up to about ten pounds of water weight/glycogen, carbs might stimulate appetite causing you to overeat, but they don't go straight to fat. I eat as many as 200g of carbs a day while training, as soon as I cut carbs the water weight disappears. Creatine supplementation adds water weight as well.
With strict keto, I can't eat enough cheese, dairy, meat to grow and recover muscle. It's the energy-density of fat and carbs that cause over eating. Try eating 20 boiled potatoes a day, it's quite difficult. However, a loaded potato with cheese and bacon is very easy to get 2000 calories.

Someone educate me on this because im still confused, 3 questions:
    1. - What is 75g of glucose, is it 75g of pure sugar? if thats the case is the study saying that if you drink a can of monster energy drink your insulin is more fucked than a regular persons?
if so how much and for how long? and if so can it not just be avoided by avoiding sugar or am I missing something here?
    2. - If it is really necessary to build tolerance to sugar, how often should you have these carb refeed days and what can you eat on them (how much percentage should be carb in these and what carbs exactly ), 4 months of keto and then 2 months of non-keto sounds like something I dont want to do, is there a better way, say 1 carb day every month...
    3. - Final question, if my diet is 60% fat and 35% protein with 5% for carbs do I need to have carb refeed days? Because as im understanding it protein makes glucose too but without kicking out one out of ketosis, so if that is the case would that diet be more beneficial in not making me insulin resistant?
